Job Description
PURPOSE OF POSITION:
Under the supervision and delegation from the Director of Pharmacy and / or supervisor, this position encompasses professional, clinical, supervisory and administrative work of varying degrees with latitude for independent initiative and judgment.
RESPONSIBILITIES:
1. Supervise and/or coordinate the operation of a subdivision of the pharmacy such as Unit Dose Satellite, Inpatient and Outpatient pharmacy, Controlled Substances, Drug Information, IV Admixing, lnvestigational drugs and Inventory Management of Drugs and ROBOT, manage the Automatic Dispensing Cabinets (PYXIS) or function in a supervisory capacity
2. Manage record keeping of Methadone inventory for the Outpatient Methadone Treatment Program
3. Assist Outpatient pharmacy Supervisor in OPD operations and Inventory management
4. Have a working knowledge of the Automatic Dispensing Cabinet and the CII safe
5. Coordinate pre-authorization of restricted antimicrobials by the ID service
6. Review medical records and prepare a chart with required patient information which includes working diagnosis, active orders of antimicrobial agents, culture results, serum levels and pertinent laboratory results necessary for effective ASP rounding
7. Review medical records and prepare a chart with required patient information which includes working diagnosis, active opioid orders/prescriptions and pertinent laboratory results necessary for effective OSP rounding
8. Responsible for documenting interventions in the ASP note.
9. Maintain summary records of interventions, ASP/OSP metrics, DoT data and ASP/OSP performance improvement
10. Knowledgeable of Federal, State and local laws concerning the filling and dispensing of medications and orders.
11. Have the skills and knowledge to provide Patient counseling and education to outpatient and discharge patients
12. Has the knowledge to review drug-drug, drug-food and drug-lab interactions.
13. To be able to fill unit dose cassettes and managing the medication cart fill process
14. Review and Profile drug orders.
15. Able to communicate well with other health professionals.
16. Suggest and Intervene on Problematic Drug Orders.
17. Skills and knowledge to prepare IVs.
18. Has the Knowledge to provide up-to-date drug information.
19. Assist in maintaining accurate and complete records and statistics of departmental activities.
Minimum Qualifications
1. Possession of a valid NY State license to practice pharmacy.
2. Two years of post licensure experience; or
3. Completion of a post graduate specialty residency or fellowship program; or
4. Current Pharmacy Board Certification.
