An advanced practice provider that will work in the Vascular surgery inpatient floor, that will provide expert knowledge in the assessment, planning, implementation and evaluation of care for the vascular surgery patient. The Vascular APP will also assist in facilitating service activities, including inservice education, patient/family education, performance improvement, and outreach and education as needed. The Vascular APP may also be required to assist in outpatient clinic and operating room when necessary.
Participate on AM rounds M-F which includes evaluating and presenting the patient to the Attending.
- Includes being familiar with vitals, labs, and radiologic exams.
- Keeping track of all consulting service recommendations and relevant follow-up.
- Provide assistance in daily progress notes and discharge summary.
- Place consults in a timely fashion.
- Follows up throughout the day on labs, consults, additional studies
Update and manage problem list in the EMR and on the patient list
Communicate with the patient and family and update them on plans of care along with any changes (positive and negative) in condition or management.
Communicate plans of care to the nursing staff daily, including plans for hospital discharge.
Sign out pertinent patient issues to Vascular Surgery Attending prior to leaving for the day.
