About the Role:
You will play a vital role in delivering compassionate and effective care to our patients in post-acute care communities. Your responsibilities include:
- Providing comprehensive primary care services to a manageable patient panel of 15-20 patients per day, 4 days per week. Or, 20-25 patients per day, 3 days per week.
- Collaborating with interdisciplinary teams to optimize patient outcomes
- Ensuring smooth transitions of care with hospice, palliative care, and home care agencies
- Maintaining accurate and timely documentation in our EMR system (Gehrimed)

- Strong clinical assessment and diagnostic skills

- Experience in post-acute care preferred, but not required (training available)
