ICU RN - Per Diem*

*IMPORTANT NOTE: In lieu of benefits due to "per diem" status, 15% will be added to the hourly rate. Per diem employees are offered work on an "as-needed" basis.

POSITION SUMMARY:

Provides professional nursing care to patients utilizing established policies and procedures ensuring safe and effective care; provides age appropriate care from infant through the geriatric patient based on the population served; assesses, plans, implements and evaluates care in collaboration with the multidisciplinary care team; demonstrates the knowledge and skills necessary to Identify and meet the individual needs of all patients served.

POSITION REQUIREMENTS:

Minimum Education:
  • Minimum of an Associate's Degree in Nursing.

License Required:
  • Current Nevada RN license required.

Certificate Required:
  • BLS for Healthcare Providers required.
  • ACLS certificate required.

Minimum Work Experience:
  • Professional customer service skills.
  • Ability to read and communicate in English, bilingual abilities preferred.
  • Minimum of 6 months ICU experience, 2 years' ICU experience preferred.


POSITION ESSENTIAL FUNCTIONS:

Patient Care
  • Accurately assesses and documents patient and family needs in order to provide quality care to assigned patients and ensure integration of information among the multi-disciplinary health care team.
  • Demonstrates timely, accurate and complete EHR and/or paper documentation in accordance with specific unit standards, including but not limited to:
      • Completes patient documentation per "Nursing Documentation: Policy
      • Admission Assessment
      • Medication reconciliation.
      • Medication Administration Record (MAR) and narcotic waste per policy when applicable.
      • Risk assessments.
      • Document flow-sheets.
    • Care Plan/ Plan of Care - appropriate nursing diagnoses and/or updates and education provided to patient and/or care advocate(s) and responses of patient and/or advocate(s) to education.
    • Progress notes for variances.
    • Pain re-assessment within 1 hour of intervention.
    • Immunization status and updates.
    • Discharge planning and education for disease process, medications, DME's, and follow-up care. Completes inter agency referral forms completely and accurately if applicable.
    • Secures valuables and/or home medications per policy and releases valuables and/or medications per policy upon discharge.
    • Using SBAR communication, notifies physician of changes in patient status.
    • Notifies physician of abnormal laboratory and/or test results.
    • Completes bedside hand-off communication with all members of the multi-disciplinary team as appropriate and documents in her.
    • Performs consistent, purposeful hourly rounding.


Judgment
  • Demonstrates the skills and judgment necessary to implement medical plan of care, nursing interventions, and procedures as necessary for the care of the patient.
  • Demonstrates knowledge of hospital policies and procedures.
  • Demonstrates knowledge of commonly used meds: action, dose, and side effects.
  • Observes ten R's for administering meds: right patient, drug, dose, route, time, reason, documentation, right history & assessment of drug-drug and/or drug, food interactions and allergies, right education and information and right of refusal after thoroughly explaining purpose, desired effects and side effects.
  • Demonstrates proper procedures in all methods of medication administration.
  • Administers blood and blood products following hospital policy and procedures.
  • Initiates and maintains IV access according to hospital policy and procedures.
  • Assesses and documents IV site per unit standard.
  • Uses critical thinking and efficient time management.


Scope of Service
  • Performs all responsibilities/duties required by unit as defined in the scope of service, to ensure that the unique nature of the client is addressed. This includes, but is not limited to the age of the client served.
  • Assesses, evaluates, and carries out nursing interventions and teaching as applicable for med/surg, tele, and ICU patients, and patients receiving transfusions and IV therapy, nerve blocks for pain control, and phlebotomies.
  • Demonstrates skill and knowledge in nursing management of emergency situations.
  • Demonstrates knowledge and skill in use of patient equipment such as IV pump, Blood pump, vital sign machine PCA, Cardiac monitor, and any other equipment required for patient care.
  • Demonstrates knowledge of setting up and maintaining sterile field for minor sterile procedures such as insertion of central lines, chest tubes, etc.
  • Demonstrates knowledge of current policies and procedures pertaining to vaccination of patients and the documentation of teaching done.
  • Demonstrates knowledge and skill in specific areas of clinical experience, as applicable.
  • Completes crash cart checks and documentation as requested by charge RN.
  • Understands and can differentiate between various admission types: observation, Inpatient, and swing status.
