Title:Supervising Care Manager

Reports To: Director of Social Services

Summary: Responsible for overseeing the DAS Case Management Program and MSSP Program, to avoid premature placement of persons in nursing facilities, while fostering independent living in the community.

Essential Functions:
Oversees the Case Management Program with DAS and MSSP Program with CDA.
Trains, supervises, and evaluates program staff.
Under the direction of the Director of Social Services, the SCM is responsible for providing clinical oversight to ensure all care management activities and documentation meet CDA and DAS Standard Agreement and Waiver requirements.
The SCM position combines both administrative duties and clinical supervision responsibilities.

a. Supervises care management staff.

b. Recommends and implements policy changes.

c. Community outreach.

d. Reviews and approves all participant care plans.

e. Verifies monthly documentation of care management activities.

f. Oversight and authorization of purchases utilizing Waiver Services including verification that

all other resources have been exhausted.

g. Monitors and tracks Waiver expenditures.

h. Manages vendor issues.

i. Other resources have been exhausted.

j. Implements and evaluates the effectiveness of site Quality Assurance processes to ensure continuous quality improvement, in collaboration with the Site Director.
Maintains complete client files and completes all required data collection forms and reports.
Participates in evaluation and quality assurance activities to determine the effectiveness of the Social Services Programs as required.
Assumes responsibility for own professional growth and continuing education to upgrade case management skills; provides input to the Director in developing the training plan to meet individual as well as the case management staff's training needs.
Participates in case conferences, in-service trainings and other meetings as required.
Networks with and conducts outreach to community-based organizations, other service providers, and families and caregivers; attends and makes presentations at community meetings and other appropriate venues to inform the community about the agency's services.
Performs other duties as assigned.

Qualifications:
Master's degree in social work, nursing, psychology, counseling, gerontology, sociology, or rehabilitation and two years' experience working directly with frail older adults.
The candidate should demonstrate expertise and ability as a care manager, as evidenced by experience successfully carrying out these job duties and ability to supervise and train staff
Knowledge, skills, and experience necessary to assess the consumer's needs for services and to perform the core functions of case management.
Sensitive to the needs of the elderly and disabled adults from diverse ethnic and minority cultures.
Excellent communication and interpersonal skills.
Must be organized and able to multi-task.
Able to work independently and in a team setting.
Bilingual and bi-literate in English and Chinese.
Must be able to travel via public transportation or personal vehicle for the performance of job duties. If driving, must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage as specified in Self-Help's policy.
