JOB DESCRIPTION
As a clinical pharmacist, it's important to align pharmacy programs with policies and regulations. An evaluation of a patient's records plays an important role in determining the appropriateness of medication therapy. To ensure patients are treated appropriately, it is also important to evaluate their condition. Aspects of the role include consultation on dosages, medication substances, and advice on proper drug administration. To determine the effectiveness of medication plans, it is crucial to assess the results of pharmaceutical treatments. It is imperative to collaborate with other healthcare professionals to ensure patients receive the best care. Maintaining accurate documentation of medication plans and patient progress is also imperative to ensure that progress is being made and any issues are being addressed.

DUTIES
- Develop clinical pharmacy programs in accordance to policies and regulations.
- Review records of patients to determine the appropriateness of medication therapy.
- Evaluate the patient's condition to ensure all issues are in the process of being treated.
- Identify untreated health problems and refer patients to appropriate physicians or medical professionals.
- Develop effective medication plans that minimize the risk of adverse side effects or possible complications.
- Consult on dosages, medication substances, etc.
- Advise on the correct administration of drugs.
- Assess the results of pharmaceutical treatments.
- Collaborate with healthcare professionals to ensure optimal patient care.
- Keep accurate documentation of medication plans and patient progress.
- And other duties as required.
EDUCATION AND QUALIFICATION REQUIREMENTS
- Graduate from an accredited school of pharmacy in the United States.
- California Pharmacist Registered. (Must have upon hire)
- Completion of 1-2 years Accredited ASHP residency program-OR-Pharmacy Board Certification
- 2 years of Hospital pharmacy experience.

HOURS OF OPERATION
Full-Time
COMPENSATION
- $140,000-235,000 Annually
