Pit River Health Service, Inc.(PRHS) is a non-profit 501(c)(3) ambulatory health clinic first established in 1979. The purpose of PRHS is to serve the community and to primarily focus on the American Indian population. Services offered: Medical, Dental, and behavioral health services, transportation, outreach, and senior nutrition services. PRHS has two clinics, one is located at the base of the Burney Mountain in Burney,Ca and the other is located on the Pit River Tribal reservation in Alturas, Ca. We are open to take Native and non-Native patients and accept Medi-Cal, Partnership, Medicare, and most private insurances. We value all our patients and as our mission statement says we provide the highest quality healthcare services to them.
Position Summary: The Outreach Manager RN provides public health nursing services to individuals, families, and population groups. Nursing actions are directed toward the goals of prevention, assessment, risk reduction, and health status improvement for individuals, families and communities based on professional nursing and public health principles, concepts, and practices. Nursing activities include, but are not limited to, systematic analysis of health data, care plan development, health education and advocacy, coalition building, and collaborating with community partners to promote the health of a population.
Duties:
Specific Duties and Responsibilities: Under general supervision and/or as part of various workgroups and teams, the Registered Nurse performs the following essential functions within the framework of the Outreach Department's provision of the core functions and essential services of public health. The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. 1.Policies and procedures shall be developed, implemented, updated and revised as is appropriate, but no less than annually and submitted to the Chief Executive Director and Board for review and approval. 2.In collaboration with the Outreach Dept. Director complete the scope of work for the Community Health Service, Outreach, and Diabetes programs for the target populations. 3.Develop and implement individual and family care plans, often in conjunction with clinic medical staff, as needed. a.Performs comprehensive individual and family assessments which include health history, physical assessment, growth monitoring, and developmental assessment. b.Provides baseline assessment and appropriate referral for issues including psycho-social family functioning, substance abuse or domestic violence, and basic needs including food, housing, income, resources and supports, and access to health care. c.Evaluates outcomes of nursing interventions; works with others (clients and other professionals) to makes changes as necessary. 4.Provides care coordination, referral and follow-up to individuals and families who are members of a vulnerable population and/or high-risk groups. a.Assist with medical referrals of newborns, infants, children and OB patients. Refer parents to perinatal and childhood education programs. b.Evaluates outcomes, effectiveness of plan, and makes changes as necessary. 5.Assess and evaluate patient's response 6.Advise patients on available and appropriate medical and social services available. 7.Provide health related counseling on the prevention of communicable diseases; coordinate health programs with outside organizations and agencies; introduce general health related information and instruction to individuals, families and community groups. 8.May administer a variety of pre-examination tests including skin tests, blood tests, and other tests used in determining communicable disease; provide information and education to patients before and after tests as necessary.
Qualifications:
1.Bachelor's degree from an accredited college or university with major course work in nursing, preferred. OR 2.Five years registered nursing experience including one year of community health nursing experience, preferably within the last five years from an accredited college or university. 3.Licensed as Registered Nurse. 4.Certified Diabetes Educator (CDE) desired, three years experience or equivalent, at least one year in supervisory role. 5.Demonstrates knowledge of applicable state professional practice guidelines for registered professional nurses, the American Nurses' Association Scope and Standards of Practice for Public Health Nursing any other Federal and State laws and regulations applicable to practice as an public health nursing professional. 6.Demonstrates knowledge of research techniques and principles as well as quality assurance/improvement systems. 7.Demonstrates knowledge of data collection, analysis and interpretation techniques. 8.Ability to work with groups including facilitation, collaboration, negotiation and conflict resolution. 9.Excellent communication skills, follow instructions, and perform duties independently. 10.Clean driving record and valid CA Drivers license. Willing to travel. 11.Ability to work with Medical, Dental, Behavioral Health departments for Outreach target population. 12.Ability to establish and maintain effective working relationships with employees, general public, other departments, agencies and state officials and work with person with varying levels of education, understanding and values in a culturally sensitive manner. 13.Ability to apply common sense understanding to carry out instructions in written, oral, or diagram form. 14.Ability to deal with problems involving several concrete variables in standardized situations. 15.Willing cross train within department. 16.Possess a current CPR card. 17.Must be willing to submit to and pass a pre-employment drug screening test. 18.Preference will be given to Native Americans in accordance with P.L. 93-638.
