Transitions Home Health Care and Transitions Wellness & Therapy Care (Outpatient Therapy)

Job Summary:

Evaluates, plans, and administers comprehensive physical therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative or maintenance physical therapy as prescribed by a physician.

Level of Responsibility:

Works under the supervision of the Clinical Manager/Therapy Manager or their designee. Supervises physical therapy assistants as assigned.

Duties and Responsibilities:
  • Ongoing interdisciplinary assessment of the patient.
  • Develops, initiates, and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver (s).
  • Provides skilled therapy services per physician orders, the patient's plan of care, agency policies and professional standards.
  • Performs all aspects of patient/client care in an environment that optimizes safety.
  • Ensures that changes in patients' clinical status are communicated to other healthcare team members and agency office staff in a timely manner.
  • Obtains all necessary physician orders and collaborates with the physician as the patient's needs dictate. Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
  • Coordinate's patient care and participates in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
  • Educates and counsels patients, their families/caregivers, and other staff as appropriate, in appropriate speech therapy modalities.
  • Provides, modifies, or discontinues physical therapy modalities based upon an ongoing assessment/reassessment of the patient's clinical status and in accordance with physician orders.
  • Assesses the patient for the need for durable medical equipment as well as other adaptive equipment needs.
  • Develops appropriate home exercise programs tailored to the patient's clinical status, needs and age.
  • Provides supervision and mentorship to PT assistants.
  • Documents all care and services provided according to professional standards and agency policy and procedures.
  • Adheres to all agency policies and procedures.
  • Completes all mandatory agency training and in-service activities.
  • Participates in the agency's quality management/QAPI activities.

  • Minimum Qualifications:
  • Licensed as a Physical Therapist in the State of Colorado.
  • Graduate of a Physical Therapist curriculum approved by the Commission on Accreditation in Physical Therapy, or if educated outside the United States, graduated from an education program determined to be substantially equivalent to Physical Therapist Assistant entry-level education in the United States by a credential's evaluation organization approved by the American Physical Therapy Association.
  • Two years' experience as a Physical Therapist. Previous experience in the home care or outpatient therapy industry preferred.


  • Knowledge, Skills, and Abilities:
  • Knowledge of physical therapy practice, with emphasis on the provision of physical therapy services in the home environment.
  • Ability to foster constructive and therapeutic relationships with patients/clients and their families.
  • Ability to communicate effectively, both orally and in writing.


  • Special Requirements:
  • Current, valid license to practice as a Physical Therapist in the State of Colorado.
  • CPR certification.
  • Current, valid driver's license.
  • Access to a reliable automobile.
