Summary
Under general supervision is responsible for supporting Bexar County's Department of Preventative Health and Environmental Services as a Population Health Data Manager. The Population Health Data Manager will support the Director and Division Heads of Departments of Preventative Health, Behavioral and Mental Health, Environmental Services, and Agrilife as defined in the duties and responsibilities. This position will represent the County in a professional and effective manner and establish and maintain effective working relationships with co-workers, County employees, Elected/Appointed Officials, outside organizations and agencies, vendors and the general public.
The Population Health Data manager is responsible for evaluation and analysis of population health data to support and direct public health strategies using data tools and concepts including data visualization, forecasting and predictive modeling, and population and demographic modeling. The Population Health Data manager will coordinate data sharing with stakeholders and community partners to align shared goals for public and community health.

Duties and Responsibilities
- Define complex and detailed statistical analyses and modeling of data with outputs that are clear and relevant to stakeholders, such as opportunity analyses, clinical trending, operational KPIs, predictive modeling, program evaluation, program reporting, and ad-hoc analytics
- Participate in the design and development of statistical analyses and reports that explore and present large amounts of data to discover underlying patterns and trends related to chronic illness, health risk, cohort analysis, and program evaluation
- Apply research and evaluation standards, descriptive statistics, and data management principles to translate complex data into meaningful content
- Through the use of risk stratification, identify characteristics of different populations who have been made vulnerable. Develop models and understanding of community-based coordinated care
- Working with the Preventative Health and Environmental Services to design models of care to focus on specific populations to define protocols and care pathways. Analyze data to stratify the population by risk profile to design effective strategies to address health disparities and issues of inequity
- Present findings using verbal, written, and graphic communication techniques
- Offer recommendations to support organizational performance improvement
- Independently complete analysis, development, and enhancement of analytical and operational solutions to fulfill business needs
- Apply understanding of dimensional data modeling techniques, data warehousing, design, and concepts to produce balanced scorecard design, and design of dashboards and KPI's (Key Performance Indicators)
- Analyze large data sets of structured, semi-structured unstructured data to discover data insights and collaborate with business partners to deliver business value to discover trends, patterns, and actionable insights
- Design interactive and/or advanced visualizations (e.g. Tableau) that enhance analytics storytelling to support business decisions
- Navigate multiple departmental programs and draw holistic insights from large data sets.
- Represent the Department of Preventative Health and Environmental Services on internal and external committees and at County-wide meetings
- Performs related duties as assigned
Minimum Qualifications
- Graduated from an accredited college or university with a minimum master's degree (doctorate preferred) in demography, epidemiology, data science, or public health or closely related field, and seven (7) years data analysis experience to include two (2) years supervisory experience and or an equivalent combination of education and experience.
- Ability to manage and train employees; ability recognizing problems and implement solutions to resolve; ability to communicate effectively, both verbally and in writing; ability to communicate effectively with diverse groups of individuals utilizing tact and diplomacy; ability to establish and maintain effective working relationships with subordinates, co-workers, County employees, Department/Division Heads, Elected/Appointed Officials, outside organizations and agencies, vendors and the general public
- Strong organizational skills that reflect ability to perform and prioritize multiple tasks seamlessly with excellent attention to detail; skill in supervising professional employees and maintain confidential information; skill in operating and maintaining office computer systems and a wide variety of office software packages
- Must secure and maintain a favorable background investigation and clearance
