The mission of the Americares Free Clinics is to provide quality health care to low-income, uninsured patients in a setting where all individuals are treated with dignity and respect. Americares opened its first clinic in 1994 to expand access to care for the working poor. Over the years, our program has grown to become the largest free clinic network in the state with four locations serving 2,000 patients annually. Today, our clinics in Bridgeport, Danbury, Norwalk and Stamford are the primary source of free medical care for the uninsured in Fairfield County.

About the position:

Americares Free Clinics provides comprehensive primary health care to an adult population of low income, uninsured residents living in the clinic's service area. Many of our patients face barriers in access to health care for both preventative visits and care of chronic conditions. This leads to poor outcomes for many of the working poor in Fairfield County.

As an Americares provider you will provide primary care to the adult population of the Norwalk Clinic most of whom are diagnosed with chronic illness such as hypertension, hyperlipidemia, obesity and/or diabetes. You will also serve as the medical director overseeing the delivery of quality care at three other free clinic locations. This position is 80-90% clinical care and 10-20% administrative duties. Instead of spending time speaking to insurance companies, you can spend your clinical time speaking/listening to your patients while working with your health care team to deliver the highest level of care.

Resources to accomplish this medical home model include laboratory testing, diagnostic testing, specialty care and the provision of medications. You will become a member of a supportive team at your clinic which includes nursing, medical assistants, a health coach and administrative staff to provide structure and care management. Medical malpractice insurance is included.

Key Outcomes:

In the first 90 days, the Medical Director/PCP will:
Learn and live the Americares values.
Complete all required new hire training, including Athena training (EMR).
Demonstrate an understanding of the patient population and demographics.
Demonstrate respect for patients and staff while delivering quality care
Demonstrate an understanding of clinic policy and procedures.
Establish meeting schedule, teaching protocol and peer review system with staff and

volunteer providers

In the first 6 months, the Medical Director/PCP will:
Demonstrate full functional competency in Athena, including fulfilling preventive care and QI guidelines
Deliver high quality medical care to patients while mentoring medical staff to optimize

health outcomes
Contribute to clinic operational flow, huddle activities and staff development.

In the first year, the Medical Director/PCP will:
Deliver consistent, comprehensive, high quality medical care to our patients
Participate in policy development as requested
Work collaboratively with team to promote new programming and approaches to

efficiency and quality care.
Work with the leadership team to ensure completion of AFC's operational plan and

goals.

Duties and Responsibilities as a Medical Director:
Conduct monthly provider meetings to review current trends in medicine, provide

continuing education on necessary topics, review complex patient care to create a

multidisciplinary approach.
Continual evaluation of and oversight of the peer review program while providing

constructive feedback, as needed.
Participate in the Quality Improvement program and be a resource to its members while

actively participating in continuous improvement.
Oversight of the performance review of medical staff and NP staff as it relates to the

provision of patient care.
Demonstrate good stewardship of clinical resources throughout practice setting
Provide feedback and annual evaluation of clinical performance of PCPs
Participate in orienting new providers to clinical protocols, EHR and policies/procedures

related to medical practice

Duties and Responsibilities as a PCP:
Participate as an active member of the medical staff/team and provide coverage and/or

advice to other AFC clinic providers
Providing continuity and completeness of medical care by obtaining appropriate patient

history, performance of a physical evaluation, ordering appropriate diagnostic tests with

interpretation, and formulate / initiate a treatment plan with appropriate follow up.
Determine patient medication needs, and order according to availability and accessibility
Review incoming reports (e.g. lab, x-ray, EKG) sign, date and follow up with staff and

patients in a timely manner.
Ensure electronic documentation is recorded in a complete, timely and orderly manner.
Utilize referrals and resources to optimize patient care and model best practices
Agreement to following the policies and procedures set forth within the manual as well as

any regulatory requirements for our clinic population.
Actively model and contribute to Americares values, work culture and mission.

Degrees, experience, and competencies/skills required for the role:
Medical degree and current Connecticut license (MD or DO) with board certification in

internal or family medicine
Experience working in primary care, chronic disease management setting with an adult

patient population
Experience with peer review, quality improvement and mentoring peer providers
Demonstrated competency of current treatment protocols and quality measures

associated with chronic and preventative care
Excellent verbal and written communication skills with ability to give and receive

constructive feedback in a manner consistent with our values.
Ability to work with a medical interpreter, as needed

US work authorization is required for this role.

These competencies would be great to have, but are not required:
Fluency in Spanish
Experience working with culturally diverse and vulnerable populations
