Cabell Huntington Hospital is looking for a dedicated Staff Pharmacist to join our team. In this role, you'll be part of a decentralized pharmacy model, which allows for more direct patient interaction and tailored medication management as staff and clinical pharmacists work together to optimize patent care. Your focus will be on ensuring safe and effective medication use for a diverse patient population and collaborating closely with healthcare teams.

Shift / FTE: Full-time, Day and Evening Shifts with rotating responsibilities in the pharmacy based on shift assignments. Shifts tend to be 10 hours in duration four days each week.

Key responsibilities may include:
  • Oversight of compounding and dispensing of medications and to inpatients and outpatients - sterile and nonsterile admixtures
  • Reviewing and verifying medication orders
  • Monitor patient medication regimens for potential interactions or adverse effects
  • Recommend therapeutic alternatives or dosage adjustments based on patient-specific factors
  • Provide pharmacokinetic monitoring for drugs requiring individual dose adjustments
  • Work closely with physicians, nurses, and other healthcare providers to optimize medication therapy
  • Ensuring compliance with pharmacy regulations and protocols
  • Maintain accurate and thorough documentation of all clinical interventions, medication reviews, and consults

  • Bachelor's degree required, PharmD. preferred
  • WV Board of Pharmacy Licensure and registration required
