Description:
Moorings Park is looking for an Occupational Therapist. The Occupational Therapist is responsible for ensuring and coordinating quality and safe delivery of home health occupational therapy services following applicable licensure/certification requirements both State and Federal. The Occupational Therapist has responsibility for the planning and therapeutic interventions of all occupational therapy cases. Occupational Therapy will be provided by a licensed Occupational Therapist or a licensed Occupational Therapy Assistant under the supervision of a licensed Occupational Therapist in compliance with the Plan of Care.

- Evaluates the patient's needs, including medical, psychosocial and environmental needs.
- Develops a clinical assessment based on evaluation/re-evaluation findings and requisition and management of equipment and supplies required to increase the clients function and independence as appropriate to care.
- Develop and implements, with physician and patient, individualized Plan of Care based on patient's physical, medical, psychosocial and environmental needs, and makes appropriate revisions as the patient's condition changes.
- Establishes appropriate realistic and measurable goals for patients to achieve, then evaluates progress towards these goals.
- Obtains initial and interim physician's orders and reports on progress and changes in client condition to physician and Clinical Manager.
- Provides Occupational Therapy treatments consisting of active range of motion/modalities, therapeutic exercise and ADL retraining in accordance with the plan of treatment.
- Performs reassessment and case review as appropriate and timely revision of the individualized care plan.
- Provides case management including planning, coordination of services, and therapeutic interventions as required, in the absence of skilled nursing.
- Supervises the Occupational Therapy Assistant through:
- Instructions on initial care plan as established by the qualified Occupational Therapist.
- Performs supervisory visits as required by the state and through review of documentation and communication.
- Provides timely documentation of supervision of the certified therapy assistant.
- Supervises the Home Health Aides through:
- Prepares the home health aide assignment sheet/written instructions indicating care and tasks to be performed.
- Communicates regularly with the Home Health Aide
- Performs and documents Home Health Aide supervisory visits no less than every 14 days to evaluate:
- Evaluated effectiveness of the Home Health Aide care provided
- Rapport with patient/family
- Infection control procedures
- Honoring patient rights
- Following Home Health Aide assignment sheet
- Revises Home Health Aide assignment sheet/written instructions as necessary
- Teaches and supervises Home Health Aides in new non-skilled tasks and procedures and documents such training.
- Completes comprehensive OASIS Recertification/Follow -Up, if Occupational Therapy is the only discipline ordered, along with other recertification paperwork as appropriate.
- Completes a comprehensive OASIS Resumption of Care Assessment along with other resumption documentation, as appropriate, if Nursing is not in on the case.
- Completes a comprehensive OASIS Discharge Assessment if last skilled discipline providing service.
Job Requirements:
- Excellent oral and written communication skills in the English language required
- During emergencies or natural disasters may be required to be on duty for the duration of the disaster.
- Is a graduate of an Occupational Therapy program and is licensed in the state of Florida.
- A current Florida driver's license, clean driving record and proof of insurance are required.
- Has 2 years' experience preferred
- Previous geriatric and home health experience preferred
