POSITION TITLE: Practice Manager

REPORTS TO: Director - Care Management

Who You Are

You are a motivated healthcare professional with a strong background in managing large healthcare practices. Your expertise in maintaining clinical documentation, understanding clinical software, analyzing data, and collaborating with clinical teams positions you as a key player. Your ability to handle multiple responsibilities and communicate effectively ensures that you contribute significantly to OnMed's mission of delivering high-quality care.

What You Will Be Doing
  • Partner with the Director of Client Management to recognize and execute new service expansion opportunities to continuously improve the patient's experience.
  • Identify and implement continuous improvement processes to enhance efficiency and effectiveness.
  • Develop and maintain standard operating procedures to ensure consistency and high-quality care within the Care Management function.
  • Actively engage in new client implementations to understand and integrate the client specific requirements into our clinical workflows.
  • Maintain client-specific community resource binders and update clinical documentation to ensure that resources are current and relevant.
  • Leverage reporting to understand the performance of the team and identify care delivery trends.
  • Organize the user acceptance testing strategy and maintain comprehensive testing documentation for software updates to ensure system reliability and functionality.
  • Support daily testing of the CareStation and respond to any support questions that may arise.
  • Perform such other duties as may be assigned by the Director - Care Management at her discretion from time to time.

Knowledge, Skills & Abilities
  • Strong technology skills in healthcare systems and software, including Athenahealth.
  • Proficient in creating and maintaining policies, procedures and clinical protocols.
  • Excellent organizational skills and attention to detail.
  • Strong communication skills, with the ability to interact with patients, staff, and clients' clinical teams.
  • Ability to perform system testing and support user acceptance testing during software releases.
  • Capable of managing multiple tasks and responsibilities in a dynamic environment.
  • Ability to work collaboratively with the other Care Managers, Care Coordinators, and other stakeholders to achieve operational goals.
  • Must be able to thrive in a changing environment.

Education & Experience
  • Bachelor's degree in Healthcare Administration
  • Minimum of five years of experience in practice management.
