Care Manager - ED (000087)

Job Title: Care Manager Job Specialty: Emergency Department (ED) Job Duration: 9 months Shift: Day - 5x7.5, 9am - 5pm Guaranteed Hours: 40 hours per week Experience: Two to three years of clinical experience in a certified home health agency, Lombardi program, and/or managed long-term care or managed acute program. Five years minimum of clinical experience as a Registered Nurse. Leadership experience preferred. License: RN licensed as a Registered Nurse in the State of New York. Certifications: PRI and Screen certification required within three months of employment. Certification in Case Management preferred. Must-Have: - Bachelor's degree required; BSN preferred- Experience with care coordination, utilization review, and discharge planning- Computer literacy- Bilingual in Spanish (Read, Write, Speak) Job Description: - Coordinate and manage patient care in a long-term care setting- Perform utilization review to ensure the efficient use of resources- Develop and implement discharge plans for patients transitioning from acute care to long-term care settings- Collaborate with various healthcare professionals to ensure comprehensive care- Maintain detailed and accurate patient records- Provide leadership and guidance to nursing staff involved in patient care coordination- Communicate effectively with patients and their families, particularly those who are Spanish-speaking.
