Description:

The certified diabetes care and education specialist (CDCES) delivers comprehensive and seamless educational and support services to patients and their families/care partners that bridge the gap and integrate clinical and self-management aspects of diabetes. The CDCES is an integral part of the interprofessional team and provides collaborative, comprehensive and person-centered care, and education conducive to behavior change and improved quality of life across the lifespan. The CDCES supports and advocates for people affected by diabetes to optimize quality care. The CDCES promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes. Participates in case management and patient care planning and goal setting, and in the development of education curricula and materials by writing original material and /or reviewing the work of others. Assists the Education Coordinator in the assessment, planning, implementation and evaluation of the Patient Centered Diabetes Education program. Collaborates with other educators in identifying the needs, developing and providing clinical programs and services. Promotes and disseminates quality diabetes information to professionals and the public following established American Diabetes Association guidelines. Credentialing as a Diabetes Care and Education Specialist or Board Certified Advanced Diabetes Management required within 1 year of hire.

Education
Bachelors Degree: Nursing (Required)

Experience
3 Years experience as a Registered Nurse (Required)
3 Years experience in Diabetes self-management education and support experience (Required)
1 Year experience in a community health or ambulatory care setting (Preferred)

License/Certifications
RN-LIC - PA Registered Nurse License (Required)
CDCES - Cert Diabetes Care and Ed Spec (Required)
