POSITION SUMMARY

The VP of Practice Management is responsible for the overall operations, planning, and organization of the Palliative Care Program and all other ancillary service lines other than hospice. This role is responsible for the leadership of a system-wide outpatient palliative care program that provides care coordination, care transition management, palliative care, and Advanced Care Planning services to patients with life-limiting, late-stage chronic illnesses. This role ensures strategic operational and fiscal growth, while positioning the organization as a market leader.

  • Designs, implements, and supervises the activities of the palliative care program providing leadership, expertise, training and education to meet program goals. Provides oversight and facilitation of program activities.
  • Assists in the development of the annual strategic planning process. Provides leadership and direction in the development and review of protocols.
  • Provides oversight for monitoring and reporting of all budget activities related to the program(s).
  • Creates and achieves project milestones and major outcomes by establishing databases for statistical monitoring and evaluation. Works closely with others in the organization to ensure accurate tracking of designated measure outcomes and prepares reports for specific target measures.
  • Develops and maintains effective working relationships necessary to carry out program goals and objectives with affiliate executives, management, staff, participating physicians and members of medical groups and foundations. Educates the community and referral partners on end of life programs, benefit coverage, and eligibility for the program(s).
  • Provides expertise in the resolution of problematic situations with patient/families, physicians and other care providers, facilities, and external entities.
  • Evaluates staffing needs and works with direct leadership to meet the needs within budgeted goals. Assist in recruitment and hiring of quality staff and provides oversight of all employees in the Practice Management department to ensure productivity. This includes direct supervision of APP's, Office support staff, and other disciplines providing care to patients. Mentors direct reports through active supervision, ride outs, one to one sessions, and skill development. Address discipline concerns and counseling with employees in collaboration with HR, when necessary.
  • Works with direct leadership to develop program policies and procedures, staff education and materials, and marketing materials.

    POSITION QUALIFICATIONS
    • Bachelor's degree required; Post Graduate degree in a medical field of study is a plus
    • Palliative care Program management experience required.
    • 3-5 years of Supervisory or program management experience.
    • Proficient skill using a computer is required, Microsoft office suite experience required.
    • Valid Texas Driver's License
    • Proof of current Automobile Liability Insurance

    PHYSICAL REQUIREMENTS:

    While performing the duties of this job, the employee will be required to stand, walk, lift, carry, push, climb, pull, bend, twist, squat, reach and kneel daily, for varying lengths of time according to each patient's need. Employee must be able to carry up to 25 pounds and lift up to 50 pounds.

    Work Environment Requirements

    While performing the duties of this job, the employee may work in various locations (homes, facilities, hospitals, Inpatient Units) and in various conditions with possible exposure to blood and bodily fluids and to infectious diseases. The job requires the ability to work a flexible schedule, ability to travel locally; PRN emergency call. While performing the duties of this job, there may be exposure to outside weather conditions when traveling between sites. Patients' home environments are variable regarding heat, noise, air circulation and conditioning, space, cleanliness, accessibility, and all other environmental factors. Inpatient and skilled nursing facility environments are standardized. Office environment is in a controlled atmosphere building.
