    • BLS Certification Required
    • Board Certified or Board Eligible in Family Medicine. If Board Eligible, must be actively working toward Board Certification.
    • Demonstrated competence in clinical assessments, pre-operative evaluations, and establishing treatment plans.

  • PART TWO: FUNCTIONAL RESPONSIBILITY
    • Position Objective
      • Provides primary care services to Adult and Pediatric individuals and their families.
    • Performance Expectation
      • Evaluate all assigned adult patients presenting to the Pre-Operative Clinic and the Primary Care Clinic.
      • Obtain an appropriate history and physical exam.
      • Order and interpret diagnostic laboratory and imaging studies.
      • Provide appropriate treatment to all assigned patients presenting to Primary Care and/or the Pre-Op Clinic.
      • Obtain consultation as clinically indicated and appropriate.
      • Document the history, physical exam, and laboratory and imaging studies as well as treatment in the medical record.
      • Obtain appropriate informed consent for all procedures as described in APD Medical Staff Bylaws, Rules and Regulations and Policies.
      • Complete all records within 7 days of day of service.
      • Provide consulting providers with complete and accurate medical records. Communicate in a timely and collegial manner.
      • Review previously ordered laboratory and imaging studies and determine appropriate course of treatment for patient based on results. Communicate results to patients in a timely manner.
      • Document all "after-hours" communication with patients.
      • Treat all patients and staff with respect.
      • Participate in staff education and clinical Quality Assurance/Performance Improvement activities.
      • Attend clinic staff meetings.
      • Abide by CMS Conditions of Participation for Hospitals.
      • Abide by the APD Medical Staff bylaws, Rules and Regulations and policies.
      • Abide by all APD policies
