Responsibilities:
The Nurse Practitioner or Physician Assistant will provide care to patients with diabetes and other endocrine disorders with attention to medication management for glucose, blood pressure and cholesterol control; other preventative care for diabetic patients; and communication with providers.
Job duties will include:
- Will develop expertise in the management of patients with diabetes and other endocrine disorders.
- Demonstrates fundamental grounding in medical theory and practice with mastery of knowledge within a defined content area.
- Manages a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care over time and across healthcare settings.
- Provides leadership in the development, validation, implementation, and evaluation of standards, policies, procedures, and critical paths to support the patient care delivery model.
- Oversees care coordination in the acute and ambulatory care settings as feasible.
- Assists the healthcare team to achieve mutually agreed upon healthcare goals by providing necessary support, education and encouragement.
- Demonstrates advanced physical and psychosocial assessment skills and knowledge of family dynamics.
- Provides direct patient care to patients in acute care and ambulatory settings.
- Performs advanced practice skills under the supervision of the appropriate physician.
- Provides expert consultation for clinical issues promoting increased knowledge and skills for the consultee.
- Participates in the education and mentoring of healthcare providers.
- May facilitate patient/family education in area of expertise.
- Annually completes mandatory safety infection control and BLS,
- Demonstrates knowledge and skills necessary to provide care to patients with consideration of aging processes, human development stages and cultural patterns in each step of the care process.
Education/Experience Requirements:
- Successful completion of advanced practitioner training and certification as a practitioner (general or specialized) from an approved Nurse Practitioner program or Physician Assistant program. Prefer two years of professional experience in a clinic setting.
Must possess a current BLS or ACLS certification and current health records with the appropriate immunizations or proof of immunity to work in the health care field (hepatitis B and tuberculosis). - Knowledge of anatomy and physiology, pathophysiology, pharmacology and therapeutics, basic nutrition, behavioral factors, psychosocial/family systems, diagnostic testing, interpretation of results and clinical decision making, alternative treatment modalities.
- Knowledge of practices and regulations related to evaluating and providing patient care, and professional ethics related to the delivery care.
- Knowledge of medical equipment and instruments used to administer patient care.
- Knowledge of common safety hazards and precautions to establish/maintain a safe work environment.
- Knowledge of health promotion, health risk identification, and patient education principles and techniques.
- Knowledge of community, professional, and educational resources.
- Skill in oral and written communication including interviewing to gather medical histories; providing presentations, consultations, facilitation, and documentation, e.g., writing medical orders, patient education materials, and medical record updates. May require skill in using computerized health information management systems.
- Skill in case management, time management, problem solving, crisis intervention, multitasking, and other organizational matters.
- Skill in modifying methods and techniques of professional nursing to provide appropriate nursing care and in initiating appropriate emergency response and investigatory procedures.
- Ability to analyze complex data and draw conclusions.
- Ability to interpret, adapt, and apply guidelines and procedures.
- Ability to establish/maintain effective working relationships with patients, clinical staff, and the public.
