Description:

SALMON Home Care is seeking a Quality Improvement Director to oversee and manage all aspects of quality, compliance, outcomes, and utilization of the home care agency.

  • Plan, lead, and oversee agency projects including but not limited to: process improvement initiatives, QAPI initiatives, clinician education initiatives, etc.
  • Collect and analyze quality and audit data including benchmarking data, CMS reportable data, and monitor the organization's Outcome Based Quality Initiative (OBQI) outcomes
  • Prepare and present QAPI findings to leadership and utilizing critical judgment, works collaboratively to develop and prioritize action steps in response to findings
  • Provide oversight of the Utilization Review clinicians and Quality Assurance manager.


Qualifications:
  • Current license as a Registered Nurse in MA
  • Three years related work experience in quality management/utilization management in Home Health
  • Five years nursing experience including two years community health nursing experience
  • Knowledge of federal and state standards applicable to Home Health
  • OASIS certification (COS-C) required and Coding certification (HCS-D or BCHH-C) preferred
