We are seeking a compassionate and experienced Care Manager to join our team at Assisting Hands as a key member of our care operations team. Social Worker and/or Gerontology background preferred, but not required. Aging services experience is highly desirable, especially working with family members of older adults. The Care Manager will work directly with our owners, playing a pivotal role in overseeing the delivery of care services to our clients. The Care Manager may also be involved in other aspects of the Company's operations including sales and marketing and community relations.

Live in coastal north San Diego County (Oceanside or Carlsbad or the immediately surrounding areas)

Responsibilities include:Conduct assessments of client needs and develop individualized care plans working with clients, their families, and healthcare professionals.

Coordinate care services based on client preferences and care requirements.

Recruit, train, supervise, and evaluate caregivers to ensure delivery of exceptional care in accordance with company policies and procedures.

Conduct home visits to assess client satisfaction, monitor the effectiveness of care, and address any identified concerns or issues.

Serve as a point of contact for clients and their families, providing support, guidance, and advocacy as needed.

Participate in community networking and sales and marketing activities. Referral bonuses paid for new clients and caregivers.

Collaborate with healthcare providers, including physicians, nurses, and social workers, to coordinate and integrate care services.

Maintain accurate documentation of client assessments, care plans, and services in accordance with regulatory requirements and company standards.

Qualifications:Minimum of 3 years' experience as a care manager. LVN or RN desirable but not required.

Strong interpersonal, communication, and leadership skills with the ability to collaborate with clients, caregivers, and other office team members.

Proficiency in computer skills, including Microsoft Office applications. Experience with care management and scheduling software preferred.

Compassionate, empathetic, and client-centered approach to care management.

Valid driver's license and reliable transportation.

Compensation: $42.00 - $46.00 per hour
