Full-Time Patient-Centered Care APP Opportunity (Virtual/Hybrid Position)
LifeBridge Health is seeking a full-time virtual/hybrid Nurse Practitioner or Physician Assistant to join our LBHMG Virtual Care team. This position will directly support our LifeBridge Health Primary Care practices by providing care remotely, via telemedicine and digital tools, to patients attributed to those practices.
LifeBridge Health is a $2B, 13,000 team member healthcare system that Cares Bravely for over 2.5 million patient encounters annually throughout Maryland. We are comprised of 5 hospitals, and a network of 130 outpatient practices. Our main hospitals are Sinai Hospital, Northwest Hospital, Carroll Hospital, Levindale Hebrew Geriatric Center and Hospital, and Grace Medical Center. Our LifeBridge Health Medical Group (LBHMG) practices are in Baltimore and in surrounding counties and offer primary and specialty care services to patients within our community.
The qualified Nurse Practitioner or Physician Assistant will be part of a collaborative care model, working closely with our LifeBridge Health Medical Group primary care providers and our Center for Virtual Care leadership.
The patient-centered virtual care program aims to create a collaborative primary care experience for patients with existing LBHMG primary care providers. This position will provide increased access for patients needing same-day or low acuity visits, support patients in achieving quality outcomes, and leverage remote monitoring for certain chronic conditions.
The successful candidate will be an experienced Nurse Practitioner or Physician Assistant who is comfortable managing patients virtually. Experience with telemedicine and digital tools is a plus. This is a remote position but will require periodic on-site meetings with the primary care teams.

Requirements
- Master's degree and Advanced Practice Nurse, Nurse Practitioner license (APN, APRN, or NP), or Maryland Physician Assistant license/NCCPA Certification
- Maryland CDS & DEA license (or attainable)
- Minimum of one year NP/PA experience, with primary care, emergency medicine and/or urgent care background preferred.
- Telemedicine experience preferred.
- Ability to communicate effectively, both in writing and verbally
- Demonstrated computer proficiency.
