Family Medicine Clinical Pharmacist

Provides quality and comprehensive pharmaceutical care to our patients to assure safe and effective drug therapy. Requires independent decision making, clinical judgment, and in-depth knowledge of current pharmacotherapy literature.

Clinical Pharmacy Specialist work directly with physicians, other health care professionals and patients to ensure that prescribed medications for patients contribute to the best possible health outcomes.

Individual professional goals established annually by Clinical Pharmacy Specialist in agreement with Clinical Pharmacy Manager and will be reviewed and included as a part of the evaluation process.

Education:

• High School diploma or equivalent is required
• Bachelor's degree in Pharmacy is required
• PharmD or Master's degree is preferred
• West Virginia Board of Pharmacy Licensure is required
• Board Certified in Pharmacotherapy or other relevant Certification to specialty practice area is preferred
• Specialty Residency is preferred

Experience:

• No previous experience is required
• 1 to 2 Years of Position-Related Experience is preferred
