General Statement of Duties

Performs targeted outreach to educate and engage target populations in non-emergent resources and services, identifies opportunities to proactively engage with the County's at-risk residents, develops intervention strategies tailored to meet individual needs by providing person-centered and trauma informed case management; identifies opportunities for early interventions; develops case management plans that coordinate services across multiple County agencies and community resources and services to reduce over-utilization of 911 services for non-emergency needs; performs other duties as assigned.

Use your expertise in case coordination and community partnerships to reduce 911 overutilization and enhance service delivery across multiple County agencies and resources.

Minimum Qualifications
Four (4) year degree in a relevant field such as social work, relevant master's level degree preferred; and five (5) years of responsible directly related work experience, preferably with prior supervisory experience; OR: Any equivalent combination of experience and training which provides the necessary knowledge, skills and abilities.

- Supervise and guide the CARE Team, including civilian and sworn staff, ensuring effective service delivery and communication.
- Evaluate and improve CARE Team operations to increase efficiency and community impact.
- Develop and implement personalized care plans, connecting individuals with County and community resources.
- Identify trends, engage with at-risk residents, and educate stakeholders about available services and interventions.
- Build and maintain relationships with local agencies to strengthen referrals and expand program reach.
- Analyze program data to assess outcomes, identify trends, and provide insights to leadership.
- Represent the CARE Team in community meetings and initiatives, advocating for vulnerable populations.
The successful candidate will have:
- Strong background in social work, case management, or community-based services.
- Excellent knowledge of County agencies, resources, and protocols for protected health information.
- Strong communication skills, including the ability to engage with diverse communities and deliver presentations.
- Ability to analyze data, develop intervention strategies, and create actionable care plans.
- Flexibility to work in various settings, including homes and offices, with some travel required.
- Ability to manage a budget and ensure program resources are used efficiently.
- Proficiency in computer software and applications for task completion and communication.
