What You'll Do

Leadership & Team Development
  • Mentor, train, and inspire a team of care managers and support staff to perform at their best.
  • Oversee daily operations, ensuring policies are followed and compliance standards are met.
  • Coordinate staffing and scheduling to meet patient needs seamlessly.

Care Coordination & Planning
  • Design and oversee individualized care plans to optimize patient outcomes and minimize hospital readmissions.
  • Work closely with multidisciplinary teams to create holistic solutions for patients with complex conditions.
  • Monitor patient progress and refine care strategies to align with real-time data and patient feedback.

Program Development & Innovation
  • Lead initiatives to improve care management processes, ensuring patient-first solutions are always prioritized.
  • Monitor and analyze key metrics like satisfaction scores and readmission rates to drive performance improvements.
  • Oversee programs focused on care transitions, discharge planning, and chronic disease management.

Compliance & Documentation
  • Ensure all care management activities meet healthcare regulations, accreditation standards, and payer requirements.
  • Supervise accurate documentation and timely reporting to support audits and compliance efforts.

Collaboration & Partnerships
  • Build strong relationships with community resources, external providers, and insurance companies to streamline care coordination.
  • Advocate for cost-effective, patient-centered care through strategic collaboration.

Resource & Budget Management
  • Manage departmental budgets with precision, ensuring optimal resource utilization without compromising quality.

What You Bring
  • Education: Bachelor's degree in nursing, healthcare administration, or a related field (master's degree preferred).
  • Experience: At least 5 years of experience in care or case management, including 2+ years in a leadership role.
  • Skills: Proven leadership abilities, strong communication, exceptional organizational skills, and expertise in care management software.
  • Knowledge: In-depth understanding of healthcare regulations, compliance standards, and care management best practices.
