1 position is available on Fanning 2 W in the Rehabilitation and Community Transition Program (RCPT)

Physicians will be responsible for a panel of residents (roughly 10 individuals) on a subacute unit, as assigned by the medical site lead. They will be designated as the attending physician for this group of clients. The role of the attending physician is to provide access to quality medical care through strong physician and interdisciplinary support. They will be expected to work closely with site RNs, LPN, physiotherapists, occupational therapists, social worker, dietician, pharmacist as well as any other members of the interdisciplinary team. The clients are generally admitted from the ER or from an acute hospital unit in the situation where there is need for further time to strengthen/recover from an acute insult of some sort. This would include post fracture, post op rehab or post prolonged hospitalization with deconditioning, where they do not have sufficient support in their current living situation. These patients will be medically stable prior to transfer and have potential for rehabilitation. There are also clients who are on active hemodialysis, admitted for rehabilitation which is managed by the RCPT. Dialysis is managed by the dialysis unit on 3W Fanning and the nephrologist on that unit .

Duties will include the following:
Provide complete comprehensive medical assessment at the time of admission and through regular weekday visits according to a schedule agreed upon by the medical site lead and participating physicians.
Complete medication reviews, review behavior maps and participate in Goals of Care conversations as required.
Participate in as required interdisciplinary discharge planning conferences.
Provide legible documentation in the progress notes of all encounters and provide written and verbal orders.
Collaborate with the multidisciplinary team to attain highest level of function possible to allow for discharge to home or to an appropriate supported level care.
Collaborate with specialty teams providing consultation to sites such as Geriatric Mental Health
Respond to concerns from nursing and other members of the care team during regular working hours. When present at the facility the physician will provide care for additional residents identified by the interdisciplinary or participating physicians as required.

Time requirement:
Work includes scheduled rounds- at least 3 times per week but dependent on patient need. The usual is 4- 5 visits per week until patients are listed for long term care in those cases where rehabilitation efforts to return the patient to pre-admission home setting is unsuccessful. There will be weekend admissions and rounds during on call periods.
Provide after hour coverage (evenings and weekends ) through participation in the afterhours call schedule. When present at the facility the physician will provide care for additional residents identified by the interdisciplinary or participating physicians as required.

Qualifications

  • Must have a family medicine designation with the College of Family physicians of Canada
  • Eligible for licensure and in good standing with the College of Physicians and Surgeons of Alberta and CMPA
  • Eligible for privileges within the Department of Family Medicine, Alberta Health Services
  • Clinical excellence in the described role, with previous experience with managing geriatric syndromes and older adults with mental health issues being an asset
  • Excellent clinical, interpersonal and communication skills
  • Demonstrated ability to collaborate in a team setting to create patient focused solutions
  • Exhibits leadership skills and potential
  • Desire to participate and contribute to quality improvement activities
  • Will be required to attend an in-person interview with an interview panel and provide references upon request

The candidate must be eligible for Licensure with the College of Physicians and Surgeons of Alberta.
