The Carewest Glenmore Regional Community Transition Program (RCTP) Unit is a 35-bed inpatient unit located at the Carewest Glenmore Park Centre. It provides patient-centered specialized rehabilitation to individuals with impairments resulting from their acute medical conditions and/or activity limitations from their surgeries in the transition, subacute setting. Family physicians are part of the expert team in the rehabilitation of patients undergoing convalescence from their acute medical conditions and/or recovery from their surgeries. This team includes nurses and allied health care professionals (e.g. pharmacists, physiotherapists, occupational therapists, social workers). The team provides assessment, individualized and group treatment, consultation, education and home assessment services to facilitate the patient's recovery of function and their safe re-integration to their community.

The Family Physician works closely with the team to support the patients through the course of inpatient rehabilitation. Visiting mental health team (psychiatrist and psychiatry nurse) and pain management specialist are also available for consultation. There is on-call coverage 1 in 4 weeks throughout the year.

The Family Physician is the most responsible physician for the patient. The Family Physician admits the patient and provides daily scheduled care including response to any changes in patient condition which require urgent attention. After inpatient rehabilitation is completed, the Family Physician provides a timely and full discharge summary communicating the course of inpatient care, patient status and required follow up for successful reintegration into the community.

Qualifications

Qualifications:

  • Eligible for coverage and in good standing with the Canadian Medical Protection Agency
  • Eligible for privileges within the Department of Family Medicine, Alberta Health Services
  • Previous inpatient experience required
  • Prior experience in managing complex medical and post-surgical inpatients an asset
  • Prior experience in rehabilitative management of patients in the transition, subacute setting an asset
  • Excellent interpersonal, communication and leadership skills
  • Eligible for licensure and in good standing with the College of Physicians and Surgeons of Alberta
  • Eligible for certification by and in good standing with the College of Family Physicians of Canada
  • Demonstrated ability to collaborate to create patient focused solutions
  • Proven ability to work well within a team setting
