### Must-Have:
- Experience with EPIC
- Ability to complete assessments on all admitted patients
- Skills to attend interdisciplinary rounds
- Capability to initiate referrals for home health, DME, skilled nursing facilities, LTACHS, and IRFS
- Flexibility to work every other weekend
- Ability to comply with flu vaccine requirements, with medical/religious exemptions as applicable
### Description:
This full-time position requires a dedicated Registered Nurse to function as a Case Manager, focusing on care coordination across various units, excluding psych and women and infants' units. The ideal candidate will manage a caseload that includes tasks like conducting patient assessments, partaking in interdisciplinary team meetings, and initiating necessary referrals for post-discharge care services. A thorough understanding of EPIC systems and previous involvement in facilities of similar size is crucial for success in this role. Additional duties include fulfilling weekend and holiday obligations as required. Candidates will float within the service line as needed, helping to ensure comprehensive and coordinated patient care. Orientation consists of 40 hours. The role allows for continued learning and development through pre-start modules, which are obligatory and designed to enhance clinical skills.
