Responsibilities
Patient Care: Provide high-quality, patient-centered care, including preventive services, diagnosis, and management of acute and chronic conditions.
Coordination of Services: Serve as a "health manager" by coordinating patient care, guiding diagnostic procedures, and facilitating referrals to specialists when necessary.
Health Education: Counsel and educate patients on safe health behaviors, self-care skills, treatment options, and preventive healthcare services.
Collaboration: Work collaboratively with a multidisciplinary team to ensure comprehensive and continuous care for patients.
Qualifications
Medical degree from an accredited institution.
Board certification or eligibility in Family Medicine, Internal Medicine, or related field.
Valid medical license to practice in the state of Georgia.
Excellent communication and interpersonal skills.
Commitment to providing compassionate and patient-centered care.
