Schodack Internal Medicine and Pediatrics is seeking a full time Clinical Pharmacist. This is a 40-hour work week, Monday-Friday.
The Clinical Pharmacist will oversee comprehensive medication therapy management, identifying patients needing intervention and addressing complex therapeutic challenges such as statin intolerances and blood pressure management. They will collaborate with clinical teams to ensure smooth care transitions, conducting medication reconciliations and addressing barriers to adherence. The role involves providing drug information to healthcare staff, developing patient-specific care plans, and ensuring population-based clinical goals are met, including vaccination and routine care updates. Additionally, the pharmacist will manage medication affordability, establish office workflows for disease management, and serve as the primary contact for pharmaceutical companies, ensuring the dissemination of accurate drug information and the proper handling of medication samples.

Annual Salary Range: $110,000.00-$153,000.00

Essential Functions include, but are not limited to:
- Medication Therapy Management
- Identify population of patients where pharmacist intervention is needed
- Determine selection criteria: i.e. X number of chronic medications, certain identified chronic diseases, etc.
- Therapeutic options for provider identified patients: i.e. Multiple statin intolerances, BP management strategies, monitoring and adjusting insulin therapy, etc.
- Assist with coordination of care at all points of transition of care
- Complete medication reconciliation with patient/care givers following hospital discharge, rehab discharge, etc.
- Identify for any potential barriers to medication treatment plan
- Collaborate with clinical staff to ensure patient needs are met
- Drug information questions from providers, nurses and patients
- Drug updates
- Adverse events
- Dosing recommendations
- Interactions
- Population based team reviews- i.e. DM patients
- Attaining clinical goals. Medication initiation or titration needed.
- Appropriate and timely disease and drug monitoring
- Is patient up to date with vaccinations
- Have self-monitoring devices been ordered
- Up to date with routine care. Ie. Eye exams, BMD, podiatry, etc.
- Develop patient specific care plan with team
- Medication adherence and affordability
- Medication reconciliation
- Cost effective therapeutic alternatives (as appropriate) or resources that may assist in obtaining medication
- Development of office-based workflows/algorithms for disease management
- Academic detailing
- Act as the providers primary source of drug information, new drug updates, warnings, disease management, new clinical updates/guidelines, etc.
- Clinical Pharmacist to act as primary contact for pharmaceutical companies
- Filter information coming in and disseminate appropriate info/resources to providers
- Streamline, manage and maintain samples
- Limit number and type of samples on site (decide this with providers)
- Log samples coming in (lot #, exp date) and samples going out (pt name, DOB, MRN, drug, dose, quantity, lot #, exp date, etc.)
- Train clinical staff regarding medication use, therapeutic alternatives, etc.

- Doctor of Pharmacy degree, completion of post-doctoral residency (PGY-2 preferred), or at least two years of clinical experience in ambulatory care.
- Enhanced credentials preferred (or willingness to obtain such credentials, i.e. BCGP, BCACP), eligibility for licensure in New York state.
- Immunization certification desirable.
