Alhambra Hospital Medical Center, a community acute hospital, is located about 20 minutes away from Los Angeles. We are seeking an experienced Director to oversee our Case Management and Social Services departments.
The potential candidate is responsible for overseeing both Case Management and Social Services departments. The candidate is tasked with driving continuous improvement of hospital systems, ensuring patients have timely access to the most appropriate, safe, and patient-centered care across the continuum; coordinate with physicians, clinicians, health plans, and local care providers; contribute to treatment plans that balance clinical, financial, and patient needs; build outstanding relationships with internal and external stakeholders
This position requires flexibility in hours and a strong commitment to enhancing patient outcomes through efficient care coordination.
CA RN license and Bachelor's degree in Nursing. Master in Healthcare Administration or Public Health preferred. Minimum of 3 years management experience in Hospital setting is a must. Previous experience in Case Management and Social Services required.
