The Director Practice & Provider Engagement (DPPE) will play a key role in the development and implementation of key strategic clinical initiatives, advancement of SoNE's Population Health initiatives, improving the engagement with providers and practices, and engagement, retention and expansion of SoNE's physician network. Reporting to the Chief Population Health Officer (CPHO), the DPPE will be responsible for developing and executing effective Population Health Management strategies and supporting the development of SoNE's delivery system. Along with a provider engagement program, this role requires a combination of strategic leadership, hands-on physician engagement and practice management experience.
This role is located at the company's headquarters in Windsor, CT, and operates in a hybrid/remote environment.

Essential functions include the following:
- Population Health Management: Support the CPHO and provide oversight and guidance of population health management initiatives including but not limited to:
- Assist with the development and implementation of population health related initiatives aimed at improving clinical outcomes and reducing the total cost of care. Examples include Urgent Care strategy, Hospital at Home, Service Lines, Establishing High-Value Specialty Care and Community Hospital Networks, and other ambulatory site of care opportunities.
- Provide operational oversight of specific initiatives
- Engage with key clinical and administrative leaders and staff as needed
- Provider/Practice Engagement Strategy: Develop and implement comprehensive physician engagement strategies and Population Health Management practice improvement opportunities aligned with the healthcare organization's goals and specialties. Collaborate with senior leadership to understand current and future delivery system priorities and initiatives.
- Provider Engagement Execution: Personally engage in provider and practice engagement efforts, optimizing understanding of and participation in SoNE's strategic initiatives including Network activities and Population Health Management activities.
- Stakeholder Collaboration: Work closely with the SoNE Medical Director, and other key stakeholders to understand specialty-specific needs. Collaborate with internal teams to ensure a coordinated and efficient engagement strategy for the delivery system.
- Provider/Practice Onboarding: Support the onboarding process for recruited providers as needed, collaborating with relevant departments to ensure a seamless transition. Monitor and enhance the onboarding experience to promote provider retention and participation in SoNE initiatives.
- Provider/Practice Communications: Develop and execute marketing strategies to promote the organization as an entity of choice for providers. Collaborate with the marketing team to create compelling engagement materials and an online presence.
- Metrics and Reporting: Establish and monitor key performance indicators (KPIs) to measure the effectiveness of provider/practice engagement strategies. Provide regular reports to leadership on recruitment metrics, challenges, and successes.
- Provider/Practice Community Engagement: Build and maintain relationships with medical schools, residency programs, academic medical centers, community hospitals, and physician communities to enhance recruitment pipelines. Attending industry events and conferences to network with potential candidates.

Qualifications
- Bachelor's degree in business or healthcare administration or related field is required. Additional advanced clinical or business credentials preferred. Master's degree highly desirable.
- Five years plus practice management experience with expertise in operational aspects of primary or specialty care as well as physician network development and engagement experience and population health management
- Strong operational leadership skills, including experience providing business leadership to physicians.
- Fiscal understanding of physician practice revenue and reimbursement cycle management
- Exceptional interpersonal and relationship-building skills
- Excellent engagement and presentation skills
- Experience developing and using metrics to measure performance of teams, processes, and services.
- Strong analytical, problem solving and decision-making skills.
- Solid change management skills
- Proficiency in computer software applications (MSWord, Excel, PowerPoint)
- EMR Experience preferred.
