HealthPoint's Whole Care Program, based in Auburn, WA, was selected to become one of Washington's first Health Engagement Hubs serving people using drugs. As a Whole Care - Health Engagement Hub team we aim to provide a safe location for people using drugs to receive harm reduction, primary care, behavioral health and social services. Our interdisciplinary team works closely together and partners with patients and community organizations such as shelters, sober-living facilities, supportive housing organizations and the City of Auburn in this work. We meet patients in the clinic and in the community, wherever they feel most comfortable. This position will be a part of the Whole Care - Health Engagement Hub Team."
Position Summary:
The Substance Use Disorder Professional is responsible for providing chemical dependency screening, assessment, treatment and care management services to patients with addiction problems of broad-spectrum severity, who often exhibit co-morbid medical and psychiatric problems. This provider will function as a core member of a collaborative care team that involves the patient's primary care provider, behavioral health consultant and other providers that may be available in the primary care clinic.
Compensation is dependent on skills and experience.

Your contribution to the team includes:
- Responsible for delivering integrated health care in consultation with a primary care team.
- Provide substance abuse screening, education and services to identified patients to promote improvement in medical and mental health.
- Monitor, evaluate, and record patient progress with respect to treatment goals by documenting services rendered in person or by phone in patient electronic records.
- Coordinate care to patients and families who may have difficulty in accessing medical benefits, psychiatric and/or substance abuse treatment and community referrals, following through to ensure service efficacy.
- Provide brief counseling or psychotherapy using evidence-based techniques such as motivational interviewing, relapse prevention or other relevant skills with an overall focus on a harm-reduction model of care.
- Conduct regular caseload consultation with primary care team to communicate resulting treatment recommendations to the patient's PCP or other care team members.
- Conduct group classes and/or visits to promote skill building, education, and improved health for the patient.
- Facilitate patient engagement and follow-up in health care.
- Assist in Quality Improvement projects: review utilization data, establish outcomes to measure quality of life improvements, and cost effectiveness of population-based programming.
- Maintain good attendance, is punctual and works full scheduled shift is a condition of employment.
- Demonstrate respectful, professional and appropriate behavior that supports a team-oriented work environment.
- Maintain a safe environment for patients, visitors, staff and self.

- Perform other duties as assigned by supervisor.
SECONDARY DUTIES AND RESPONSIBILITIES
- Attend staff meetings, in-service meetings, team meetings and participate in agency committees and task force activities as required.
- Participate in community outreach events and networking opportunities so as to enhance the program's success.
- Engage in public speaking and/or write educational materials.
- Support reporting requirements per grant contracting requirements.
Must have's you'll need to be successful:
- Must be independently licensed in Washington State as at least one of the following: Substance Use Disorder Professional or dually credentialed Mental Health Counselor, or Licensed Clinical Social Worker.
- At least three (3) years of experience required or lives
experience of addiction treatment experience working in addiction treatment working in outreach, an
ambulatory clinic setting or equivalent combination of education and experience that includes, working
knowledge of and experience with differential diagnosis of common mental/substance use disorders, evidencebase
psychosocial treatments for substance use disorders, brief, structured counseling techniques (e.g.
Motivational Interviewing, Relapse Prevention); or equivalent combination of education and experience. Those
with lived experience are highly encouraged to apply. Experience working with Medication for opioid use
disorder and Harm Reduction approaches. Ability to effectively communicate with primary medical care
providers about mental health and substance abuse issues, engage patients in a therapeutic relationship and
collaborate effectively in a team setting. - Ability to read, analyze, and interpret common industry related journals, financial reports and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to write reports and articles for publication that conforms to prescribed style and format. Ability to effectively present information to top management, patients, external groups.
- Position requires independent discretion and judgment. Work situations are broadly defined, complex and diverse, occasionally unprecedented. Problems have many dimensions and range from innovative thinking in one area to related fields of specialization. Consequences normally evaluated use more formal analytical, planning or process methods and strategies developed for action.
- Demonstrated operating knowledge of computers. Intermediate level ability with Word, Excel, Outlook and other required software programs.
