Job Description:
- Use the Academy of Nutrition and Dietetics Standards of Care and Nutrition Care Process, along with hospital and departmental guidelines, to facilitate the nutrition care of patients
- Implement evidence-based practices when available
- Screen patients for nutritional risk following departmental guidelines
- Conduct comprehensive nutritional assessments and re-assessments across the age spectrum
- Utilize data such as patient interviews, lab values, medication impact, medical history, and care team notes for assessment and nutrition diagnosis
- Develop nutrition care plans, interventions, and goals based on desired patient outcomes
- Assess calorie, protein, and micronutrient needs; evaluate the appropriateness of enteral or parenteral nutrition
- Implement feeding regimes per department and hospital policy
- Educate and communicate to patients, families, and care teams about plans of care
- Monitor patient response to nutrition care plans and revise as necessary
- Document in the medical record (EMR) using the PESS format where appropriate
- Serve as a member of the nutrition support team
- Coordinate nutrition care with other team members as needed
- Utilize age-appropriate teaching strategies to educate patients and families about medical nutrition therapy
- Consider physical, emotional, and cognitive factors affecting learning ability and readiness
- Develop individual goals with patients based on preferences for learning styles
- Monitor and revise patients' progress toward goals when necessary
- Research, develop, and revise education materials as needed
- Refer patients to appropriate outpatient settings for continued nutrition care if required
- Conduct in-services and training sessions for health professionals and within the nutrition department/diet office
- Provide community nutrition education as needed and as time allows
- Act as a preceptor to dietetic interns, educating them on the responsibilities of a Clinical Dietitian and processes for nutritional care
- Identify knowledge and skill deficits in interns and provide constructive feedback and coaching
- Document all patient care activities, including evaluations, education, and progress notes, in accordance with standards of practice
- Correctly capture billing charges through accurate documentation of ongoing status, interventions, and patient responses
- Maintain effective communication with patients, families, and healthcare staff while respecting confidentiality
- Participate in professional development through department and hospital educational programs and safety trainings
- Adhere to professional codes of ethics and accreditation requirements
