Job Summary and Qualifications
As an Advanced Clinical Pharmacist, you will demonstrate proficiency in all pharmacist functions by:
- Attending and actively participating in daily multidisciplinary and bedside rounds
- Completing drug utilization review to promote rational drug therapy, leading and developing clinical services, mentoring and educating staff to ensure accurate and appropriate documentation of clinical activities.
- Conducting drug regimen reviews and responding to specific polypharmacy, pharmacokinetic and medication/MAR reconciliation consults.
- Recommending and monitoring drug therapy as assigned by the medical staff.
- Utilizing expertise to train, mentor and supervise PharmD candidates, pharmacy residents, and technicians.
- Training and participating in facility wide committees and training programs.
- Minimum of 5 years of hospital experience with 3 in Sterile Compounding field preferred.
- Completion of PGY-1 Residency preferred.
- Doctor of Pharmacy required.
- Active Pharmacy license in good standing with GA Board of Pharmacy
