INTRODUCTION

The Vice President for Population Health & Quality Assurance will partner with other senior leaders to develop and oversee strategic partnerships and projects that support, provide and/or expand services offered by Unity Health Care, Inc. in the community. The position will seek out, identify and develop new business opportunities with key partners (e.g. hospitals and managed care organizations) across Unity's service area to measurably advance population health goals. This includes working collaboratively to lead the strategic business analysis of these efforts. This position will also leverage community partnerships to increase fund development goals and community giving to the organization.

The Vice President for Population Health & Quality Assurance is also charged with overseeing quality assurance processes including systematic measurement of Unity's efforts towards the "Quintuple Aim": improved health outcomes, improved patient experiences, improved staff experiences, reduced costs, and health equity. In this capacity, the position will partner closely with Unity's Chief Quality Officer and Data Team to advance Unity's organizational Quality Improvement and Quality Assurance Plan. Emphasis will be placed on advancing team-based care (including medical assistant, nursing, and clinician collaboration) to achieve these goals.

MAJOR DUTIES/ESSENTIAL FUNCTIONS

Essential and other important responsibilities and duties include, but are not limited to, the following:
  • Develops and nurtures strategic relationships and provides oversight of business development initiatives and external partnerships; leads the development of operational processes to support and strengthen business partnerships;
  • Serves as the point of contact for to advance major initiatives within the organization and effective hand off to the appropriate implementation team.
  • Assesses how initiatives fit with Unity's mission, vision, values, and strategic plan to meet current organizational needs, and are feasible within Unity's workforce capacity and infrastructure and provides presentations, informally and formally to targeted audiences regarding Unity.
  • Provides updates routinely to the executive management team and others as needed to ensure that decisions are well-grounded and operational.
  • Maintains detailed records of all business development activities.
  • Collaborates with others to identify and prioritize new concepts for development and launch in order to support new and innovative efforts.
  • Cultivates relationships with hospitals, MCO's, or other sources in an assigned geographic area that meet the standards for expanding care or partnering with Unity.
  • Works collaboratively with strategic planning department to uncover competitive intelligence and possible opportunities for Unity.
  • Works with fund development team to advance business development opportunities and deliver various business development reports to monitor progress.
  • Develops patient experience excellence program and training.
  • Explores new markets, technology, and innovation strategy to develop insight for emerging stakeholder interests; develops innovative partnerships and programs that result in strengthened competitive advantage and revenue growth.
  • Establishes a network of internal partners to gain cross-functional leadership perspectives and garner broad business support for strategic initiatives.
  • Develops population health model positively impacting reimbursement innovation strategies and value bundling for improved patient outcomes.
  • Develops and monitor progress along a population health roadmap that is consistent across sites and involves targeting particularly minoritized and/or underserved populations.
  • Oversees population health and value-based initiatives and MCO relationships; this includes MyHealth GPS and other similar initiatives involving utilization/quality improvement strategies.
  • Introduces and develops innovative and transformation activities that measurably enhance team-based care.
  • Oversees quality assurance components of Unity's Quality Improvement and Quality Assurance Plan through the systematic measuring and monitoring of processes and feedback loops.
  • Collaborate with nurse managers to standardize care delivery processes and ensure adherence to clinical guidelines and best practices.
  • Oversee nurse-led initiatives focused on care coordination, chronic disease management, and preventive care to improve patient outcomes.
  • Develop and implement training programs for nursing staff to enhance skills in population health management, quality improvement, and patient engagement.
  • Works collaboratively with others to advance strategic goals.
  • Oversees Fundraising and Fund Development initiatives to increase financial contributions and partnerships with the organization.
  • Performs other duties as assigned.

MINIMUM QUALIFICATIONS
  • Master's degree in public health, Healthcare Administration, Nursing, Business Administration, or a related field required.
  • 8-10 years of progressive experience in healthcare preferred, including at least 3-5 years in a senior leadership role overseeing population health, quality improvement, or care management.
  • Demonstrated expertise in population health management strategies, including value-based care models, social determinants of health, and community health initiatives.
  • Strong background in data analytics and the use of healthcare data to drive decision-making and improve patient outcomes.
  • Experience leading multidisciplinary teams, including clinical and non-clinical staff, to achieve organizational goals.
  • Knowledge of FQHC-specific operations and regulatory requirements preferred.
  • Exceptional skills in strategic planning, communication, and collaboration with internal and external stakeholders.

  • Excellent outreach and communication skills are required to interact effectively with all levels of internal staff, as well as with external organizations and community potential donors.
  • Ability to work independently and collaboratively with others.
  • Demonstrated ability to synthesize, interpret, and present data to creatively identify and evaluate options.
  • Excellent strategic, systems-thinking, and project management skills.
  • Strong interpersonal skills necessary to interact with all levels of Unity personnel and with external parties, normally in situations which are sensitive and/or significant to the entire organization.
  • Skills to prioritize policy/legislative items that may have an impact on Unity's Mission.
  • Excellent written and verbal skills.
  • Ability to work under and meet deadlines.
  • Advanced computer skills, and proficiency in different computer programs (including Power Point, Excel).

RISKS

The position's work involves everyday risk and discomforts, which require normal safety precautions typical of such places as offices, meetings, training rooms, and other UHC sites. The work area is adequately lit, heated, and ventilated. All medical services shall be provided according to medically accepted community standards of care. The employee shall provide evidence of a recent (within the past twelve (12) months) health assessment that includes a PPD and/or chest x-ray results.
