Summary of Position
Key Responsibilities
Minimum Qualifications
A general knowledge of prevention strategies, such as Care Management and Quality Improvement for patients with comorbidities (chronic health conditions, behavioral health and substance abuse) and social determinants of health. Experience in primary care with an emphasis on value-based initiatives such as patient centered medical home, population health, and meaningful use. Interacts with respect and in a professional manner with patients, staff and external customers.
Experience: 5 years professional experience working in Healthcare Administration, a primary care or post-acute setting either in the capacity of a Nurse Manager or Quality Improvement Director. A minimum of 2 years in a leadership position with direct reports.
Additional skills required: Knowledge of legal and ethical standards for the delivery of primary care. Excellent training and coaching skills. Strong computer skills with knowledge of Microsoft Office products. Excellent verbal and written communication skills. Able to work independently and in a multidisciplinary team. Able to effectively utilize an electronic health record to review patient encounters. Top candidates will be highly organized and self-motivated with the ability to develop and maintain strong relationships.
Education: Bachelor's degree in nursing, healthcare administration, or related field from an accredited 4-year professional institution and a RN, NP or PA license is required.

Summary of Position
Key Responsibilities
- Provides leadership and program oversight for the Complete Health Care Management and Quality leadership across all the Alabama market
- Implements Corporate Care Management strategies, tools, technology, and workflows for Value Based Care staff in the market
- Supports data driven change and movement in Value Based Care strategies and initiatives
- Works with all key stakeholders to ensure all operational processes meet company quality standards and ensure all Value Based Care strategies are supported across Alabama clinics
- Participates in developing staffing and care designs that support the goals and priorities of value based programs
- Provides leadership and clinical insight for carrying out utilization management and PCMH activities
- Collaborates with the Nurse Manager of Value Based Care in establishing schedules, approving vacation schedules and arranging for coverage
- Manages relationships with case management, scheduling, and referral leaders for hospitalist groups, specialists, and any other external health organizations
- Reinforces continuing education, NCQA, HIPAA, and other health guidelines; as they become available
- Upholds policies/procedures relevant to PCMH, Quality Improvement, Risk Management, and Population Health.
- Participates in the planning, development and monitoring of the Alabama Value Based Care Department's annual budget.
- Attends Medical Team meetings, Medical Team Leadership Group meetings, Management meetings, and Provider meetings.
- Leads risk management, concurrent review, and credentialing exercises in collaboration with the Medical Director
