Overview
The Certified Diabetes Care and Education Specialist (CDCES) assesses the person living with diabetes in the areas of diabetes self-management education (DSME) skills and knowledge, nutritional status, co-morbid conditions, preventative care, and emotional and behavioral care, support, and wellness. The CDCES provides appropriate DSME, care, training, and support in conjunction with the interdisciplinary care team. Provides community education in the covered areas listed above to residents, family members and staff as appropriate.

Responsibilities:
Employee aids in the orientation of new employees
Employee has the knowledge base to assess and provide for DSME and Medical Nutritional Therapy
Employee has the knowledge base to formulate a diabetes self-management education and nutritional care plan
Performs MDS documentation according to State and Federal regulations.
Demonstrates knowledge of drug and nutrient interaction
Incorporates patient's cultural and religious needs into plan of care
Educates patient's/family members/staff on diabetes self-management strategies, patient centered goals and medical nutrition therapy plan of action
Maintains up to date care plans
Participates in monthly service line meetings
Keeps accurate departmental records on all patients
Performs department specific audits and creates/contributes to yearly CQI and Performance Improvement Projects
Qualifications:
Baccalaureate or Master's degree approved by the American Dietetic
Association Registered with the Commission of Dietetics Certification with New York State
Certification as a Diabetes Care and Education Specialist (CDCES)
Board Certified in Advanced Diabetes Management (preferred)
Knowledge and experience in Insulin Pump Therapy
Knowledge and experience in continuous glucose monitoring systems
Minimum of 5 years' experience in similar position.
Posted Salary Range
USD $46.56 - USD $57.75 /Hr.
