Job Description
- Evaluation of rehabilitation potential of patients referred to the institution on a timely basis.
- Timely review for each stage of referred patients and provides efficient decision-making and guidance to the care transition coordinators (CTCs).
- Guides the care transition coordinators on the medical stability and rehabilitation potentials of patients referred for Acute Rehabilitation in accordance with CMS and other criteria.
- Working closely with the Chief Medical Officer, VP of Post Acute Services, and consulting physiatrists and other specialties (Neurosurgery, Neurology, Orthopedics, Spine, Cancer, etc.) at acute care hospitals for service line development for inpatient rehabilitation.
- Advising the nursing and admitting medical teams on medical issues related to bed assignments as needed. When necessary, assist the admitting department on appropriate unit assignments and any particular issues requiring private rooms, O2, Hoyer lift and negative pressure needs.
- Works with inpatients' medical staff and social work department on in-house denials for existing patients by assisting with peer-to-peer sessions to obtain additional authorization as needed.
- Representing institution to physicians and care transition team to acute care hospitals. This will include travel to acute hospitals for presentations and relationship-building on occasion.
- Leading performance improvement (PI) projects relevant to care transition with the VP of Post Acute service and AVP of Quality.
- Improve performance metrics and processes to increase the operating efficiency of the department with defined benchmarks.
- Being in the weekend on-call pool with all other attending medical staff.
- Other duties related to hospital operations as needed including occasional unit coverage. Screening will be done by the other screening physician.
