Job Description

  • Develops and maintains a clinical practice with a patient care service, cooperating with medical and nursing personnel to optimize the pharmacotherapeutic aspects of patient care. Provides optimal selection of pharmacologic agents, and design of a regimen to maximize therapeutic benefit and minimize toxicity. Provides daily follow-up on the clinical effects of the regimen with adjustments as necessary to achieved desired outcomes.
  • Provides drug and clinical information to other healthcare professionals in response to specific questions relating to new evolving issues related to the care of patients with cancer. Provides professional education regarding new drugs, biologics, or technologies for specialized drug delivery. Provides a significant level of education to professionals, trainees, and patients. Responsible for monitoring and assessing performance of the learner and providing feedback using appropriate learning objectives that are aligned with the desired learning outcomes.
  • Performs pharmacokinetics dosing and monitoring for all patients on drugs where therapeutics levels are reported
  • Provides visual, spoken, and written clinical education and training to pharmacy students, pharmacists, clinical pharmacists, and residents in the Department of Pharmacy.
  • Provides managerial responsibilities for the operations of the Hematology/Oncology satellite, including process improvements, completing performance reviews for pharmacy staff in the Hematology/Oncology Satellite, and maintaining the oncology formulary. Designs, supports and implements policies to provide effective and efficient use of resources and for ensuring that budget, quality and value targets are met in collaboration with the Director of Pharmacy.
  • Develop, review, assists and maintain, in collaboration with members of the Department of Hematology/Oncology various protocols and regimens within the oncology electronic system.
  • Participates in committees as required e.g. Pharmacy and Therapeutics.
  • Provides patient education based on the needs of the patient.
  • Analyzes practice specific quality and key performance metrics (such as pathway adherence) regularly to identify opportunities for improvement.
  • Frequently follows-up and monitors practice performance of the Oncology Pharmacy in the delivery of services.
  • Works in collaboration with the oncology team and Corporate partners to design, perform and assemble information from different analyses and perspectives to address specific cancer care issues, to draw meaningful and actionable insights that inform and shape program(s) strategic decisions.
  • May prepare and present clinical review of oncology pathways to various committees.
  • Ensure correct preparation of all formulas or medications
  • Provides and documents drug counseling to all patients.
  • Regulations and standards governing the practice of pharmacy.
  • Keeps accurate logs on all protocol drugs and patient assistance drugs.
  • Ensure that standard drug supplies are in adequate supply.
  • Coordinates new drug education from drug representative.
  • Assist physicians with the monitoring and dosing of non-chemotherapeutic medications as appropriate.
  • Provide clinical drug information to meet the needs of the customers, including physicians, nurses, patients and other healthcare professionals.
  • Oversees in the training, orientation, and education of staff
  • Achieve cost savings targets and actively contributes to the pharmacy department pharmacoeconomic efforts and formulary management and assists the department leadership with maintaining budget targets.
  • Actively participate in clinical pharmacy programs and services and acts as a consultant in areas of expertise.
  • Must be able to communicate in an effective, tactful and diplomatic manner

  • Coordinate, educate and maintain investigational drug study activity specific to their area of practice and dispenses investigational drugs where needed
  • Develop and implement clinical competency and education modules for Staff Clinical Pharmacists
  • Directly supervise and instruct pharmacy residents and pharmacy students in clinical practice
  • Document quality activities to achieve established metrics to meet The Joint Commission (TJC), State and Federal requirements.
  • Execute established policies, procedures and protocols to ensure safe, appropriate, cost-effective medication therapies for patients and recommend therapeutic alternatives to medical staff as needed.
  • Prepare and collect data for MUEs, adverse drug reaction, medication error and pharmacy interventions summary reports, drug monographs for P&T for the Department Director, P&T Committee.
  • Works with nurse manager to develop/evaluate work flow and performance standards for pharmacy staff.
  • Provides leadership and serves as a role model and resource person for clinical and staff pharmacists.
  • Maintain a sustained record of contribution to the total body of knowledge in pharmacy practice through presentations, publications, and professional affiliation activities.
  • Submit material worthy for publishing in a peer reviewed journal at least every 24 months.
  • With the healthcare team, Implement and monitor patient medication therapy for achievement of desired therapeutic outcomes.
  • Documents interventions and outcomes related to drug therapy Prepares medication orders applying aseptic technique and the proper handling of Hazardous drugs.
  • Ensures proper storage, handling and disposal of hazardous drugs
  • Maintain accurate and complete documentation for the preparation, dispensing of preparations, ancillary supplies and infusion devices where appropriate.
  • Collaborates with the healthcare team to manage drug therapy protocols
  • Enhances the quality of care delivered to patients.
  • Supports the educational and professional development of the department.
  • Cultivates good working relationships with staff, department management, physicians and executives
  • Work with clinical end users to analyze the patient care requirements, map process models, design technical solutions to satisfy requirements, documents detailed specifications for IT build and complete formal system testing.
  • Collaborate closely with and communicate between IT leadership and the clinical community to provide leadership for clinical systems projects and initiatives.


  • Minimum Qualifications

    PharmD Pharmacy degree required
    Active New York State pharmacy license and registration.
    Active ACLS certification.
    PGY1 Pharmacy Residency completion or previous hospital experience required
    Oncology PGY2 Pharmacy Residency or a minimum of 3 years of comparable practice experience required
    Board Certification in Neuro ICU Pharmacy preferred
