The HARBORVIEW MEDICAL CENTER PHARMACY DEPARTMENT is looking for an outstanding STAFF PHARMACIST - INPATIENT/TRANSITIONS OF CARE.
WORK SCHEDULE
Full-Time/40 hours per week
Rotating/variable shift
POSITION HIGHLIGHTS
Be an integral member of the Pharmacy team and provide transition of care services to multidisciplinary teams, optimizing safe and fiscally responsible transitions of care
Responsibilities include, but are not limited to, drug distribution and transitions of care facilitation aspects of the care of Harborview patients
Strong candidates will have completed an ASHP-accredited residency program or advanced degree.
PRIMARY JOB RESPONSIBILITIES
Participate as an active member of the pharmacy team to assist in successful transitions of care for complex patients at high-risk of readmission
Identify and resolve medication discrepancies to ensure an accurate medication list at discharge for select patients
Facilitate communication with providers upon patient transition of care into the outpatient setting
Assist patients in navigating insurance coverage to minimize barriers to accessing prescribed therapies
Refer high-cost medications to outpatient billing team to manage prior authorization (PA) requests for medications, ensuring timely approval and reducing treatment delays
Participate in medication order review, clarification, validation, and order verification
Supervise inpatient pharmacy technicians in all areas
REQUIRED POSITION QUALIFICATIONS
One year of experience in pharmacy practice or completion of an American Society of Health Systems Pharmacists accredited residency training program.
LEGAL REQUIREMENT
State of Washington Pharmacist license.
