The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
GENERAL PURPOSE:
Provides individualized assistance, support, and comprehensive coordination of care for patients and their families/caregivers to eliminate barriers to timely care, facilitate the flow-through the system, facilitate interactions with the care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as the first point of contact for patients and families. Acts as a direct line supervisor to junior staff.
FT position available, primarily M-F days, evening and weekends per unit needs.
Joining the Hospital at Home team is a rare opportunity to be part of building, opening and expanding an innovative care model with an anticipated go-live date of July 1, 2025. The Nurse Navigator will play a crucial role in guiding patients through the entire continuum of care for patients participating in Hospital at Home. Responsibilities include utilizing screening tools to identify and select appropriate patients for the program. The RN navigator will work directly with patients and families to provide education on H[redacted] and successfully enroll participants. The RN navigator will be responsible to identify patient's individual needs and any potential barriers to success in the Hospital at Home program and work to mitigate these barriers. The RN navigator will coordinate the admission process for patients and be responsible to ensure smooth transition to their home setting. Must possess strong communication skills and be an independent and proactive problem solver. BSN and a minimum 3 of acute care, ICU and/or ED experience required. RN navigation or care coordination experience preferred. Valid NYS driver's license and insured personal vehicle required.
