Job Summary

Under the direction of the PCMC Centralized Patient Placement (CPP) and Transfer Center (TC) Management, the PCMC RN is responsible and accountable for coordinating optimal, safe, effective and timely bed placement for bedded patients in UCSF Medical Center. In addition, s/he is responsible for facilitating Transfer Center admissions by screening patients for clinical necessity of the transfer, determining appropriate level of care, coordinating clinical and financial information with the Financial Counselor to ensure appropriateness of the transfer, S/he is responsible for assessing level of care placement needs and giving nursing report to the admitting unit.

The PCMC nurse will act as a liaison between the Centralized Patient Placement Unit and UCSF Physicians, Nursing, Case Management and Hospitality to facilitate patient flow and timely placement of patients. Responsible for all adult area bed management and patient placement functions within the medical center. This includes, but is not limited to, patients admitted from all points of entry including the Transfer Center, ED, PACU and other portals of entry. Determines appropriate bed assignments for patients admitted to the Medical Center. This role is responsible for monitoring systemwide throughput and capacity demands and outputs while utilizing dashboards & real-time analytics to inform daily practices/standard work

Collaborates with hospital administrators, Case Management, Hospitality, nursing staff and MD's in the event of high capacity overflow to resolve bottlenecks and improve patient flow.

The flexibility to orient and work at all UCSF Medical Center locations is required.
The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The salary range for this position is $212,558 - $232,039 (Annual Rate).

Required Qualifications

  • Minimum of five (5) years recent full-time acute care, critical care or emergency RN experience
  • Previous Supervisory/Lead Experience
  • Bachelor's Degree in Nursing
  • Active Registered Nurse licensure in the State of California
  • Active American Heart Association CPR Certification
  • Experience with Epic/Apex
  • Experience with Microsoft Word, Excel
  • Demonstrated skills to effectively supervise clinicians and to manage the complex workflow and multiple priorities involved with department.
  • In depth knowledge of patient care clinical standards and care delivery, systems, external regulations and internal policies.
  • Strong analytical and critical thinking skills, with the ability to quickly analyze problems, determine appropriate level of intervention, and develop and apply effective solutions.
  • Strong interpersonal skills for effective information exchange and problem solving with all levels of management and staff, consultants and outside agencies.
  • Demonstrates an understanding of and adherence to privacy, confidentiality, and security policies and procedures related to Protected Health Information (PHI) or other sensitive and personal information.
  • Demonstrated skills to effectively supervise clinicians and to manage the complex workflow and multiple priorities involved with department.


Preferred Qualifications

  • Master's Degree
  • Strong knowledge of human resources management policies, with the ability to train, monitor, evaluate, and document human resources matters and performance, and to provide input to decision-making on human resources matters.
  • Demonstrates an understanding of and adherence to safety and infection control policies and procedures.
  • Knowledge of business software and specialized applications and data management systems used in nursing units or departments.


License/Certification

  • Active Registered Nurse licensure in the State of California
  • Active American Heart Association CPR Certification

Department Description

PCMC is open 24-hours daily as a centralized hub to support capacity & throughput across the patient care continuum. This department supports the transfer of patients to UCSF Medical Center and manages the throughput for all areas of the hospital(s), patient placement and flow to all inpatient & outpatient units. The position requires an interface with physicians, nurses and all core services in managing the admissions, transfers and discharges. This is a high volume department, one that requires flexibility, a calm demeanor and excellent communication skills. In 2023, we helped facilitate the transfer of more than 3400 patients to UCSF Medical Center and managed about 40,000 bed placements.
