Overview of Position:
Compounds and/or dispenses medications to patients and provides drug information and education to all customers as to optimize the quality of care.
ESSENTIAL DUTIES
1. Accurately performs medication order entry on computer system, assuring completeness of the patient's medication profile.
2. Provides drug and toxicological information to customers in a timely and accurate fashion in order to maximize knowledge and minimize risk/adverse events.
3. Performs accurate calculations of caloric and/or electrolyte ingredients for TPN resulting in correct administration of patients' nutritional therapy.
4. Monitors drug therapy of patients by screening for potential medication problems so that issues involving drug interactions, incompatibilities, and potential allergies are resolved without patient harm.
5. Applies pharmacokinetic/pharmacodynamic principles to individualize drug dosing for patients.
6. Contacts prescribers regarding clarification of medication orders or notifies them of unobtainable medications in order to reduce the possibility of medication errors or delay in therapy.
7. Provides educational inservice programs in order to share the latest treatments and therapies with other care providers.
8. Assists in the inventory and ordering of pharmaceuticals to minimize stock outages maintain appropriate inventory levels.
9. Assists the Purchasing Associate in procuring quality generic products as well as supports the formulary practices of the department to provide optimal care at the most affordable price.
10. Maintains medication on assigned patient care areas by initialing inspection forms and submitting reports to the manager.
11. Periodically attends nursing unit meetings to disseminate and collect information and then pass this information on to management to assure proper support and follow-up.
12. Prepares emergency carts and boxes to assure adequate quantities of in-date medication are available for emergency situations.
13. Responds to and supports Code Blue and Code Trauma situations to maximize opportunity for patient survival and stabilization.
14. Adjusts doses in patients with diminished renal or hepatic function in order to avoid potential toxicities by patients receiving too much drug.
15. Individualizes medication dosage for neonatal, pediatric and geriatric patients in order to optimize care during this phase of human development.
QUALIFICATIONS
1. Licensed Pharmacist in the State of Ohio.
2. Excellent interpersonal, decision-making, facilitation, conflict resolution and investigative skills.
3. Excellent listening, verbal and written communication skills.
4. Ability to provide outstanding customer service and maintain a high degree of confidentiality.
5. Ability to work independently and under pressure in a complex and changing working environment.
6. Excellent computer skills and knowledge of computer software, including programs such as Word, Excel, PowerPoint, etc.

WORKING CONDITIONS/PHYSICAL REQUIREMENTS

1. Works in a Pharmacy environment.
2. Employee has requirements to travel to both campuses and other locations. May be required to drive to other locations. Answers telephone calls, uses personal computer and other business machines extensively, which requires the ability to apply finger dexterity. Individual bends, reaches, pushes and pulls file drawers to file records and reports.
3. Regularly lift or move up to 10 pounds, frequently lift or move up to 25 pounds and occasionally lift or move up to 50 pounds.
4. Vision abilities required include up close vision, peripheral vision, depth perception and the ability to adjust focus.
5. May be required to periodically rotate shifts and regular days off. All system employees must be willing to work all shifts, extra hours, holidays and emergency shifts as required.
