Compensation: $140k-150k depending on experience
Hours: Mon-Fri 7:30-3:30 (flexible start time) some weekend coverage as needed

Job Summary:

Key Responsibilities:
- Oversee and develop training programs for pharmacy staff regarding policies and procedures.
- Coordinate Continuous Quality Improvement (CQI) meetings.
- Evaluate physician medication orders for appropriateness, dosage, interactions, and administration routes.
- Dispense medications by compounding, packaging, and labeling.
- Monitor drug therapies and provide advisory interventions as needed.
- Make therapy change recommendations to physicians and facility staff when appropriate.
- Handle IV dosing and administration.
- Collaborate with healthcare staff to provide clinical and technical support.
- Document all communications with physicians, nurses, and healthcare team members.
- Ensure compliance with federal and state regulatory requirements, maintaining all necessary documentation.
- Inform the Pharmacy Manager of any medication-related errors.
- Assist facility/agency staff with best practices for procuring, maintaining, administering, and disposing of medications safely and effectively.
- Perform other related tasks as assigned.
Qualifications:
- PharmD or equivalent degree required.
- Active and unrestricted state pharmacist license.
- Experience working with the geriatric population preferred.
- Strong knowledge of USP 797 guidelines.
- Ability to assess medication appropriateness and provide expert recommendations.
- Excellent communication and collaboration skills.
- Strong attention to detail and commitment to patient safety.
