Job Title: Physical Therapist - Home Health
Job Specialty: Physical Therapist

Shift: Days
Guaranteed Hours: 40 hours per week
Experience: Minimum of 1 year in Home Health
License: CA PT License

Must-Have: Experience in Home Health
Job Description:
- Required to prioritize start-of-care visits.
- Required to supervise Physical Therapy Assistants.
- Next day mandatory reporting of start-of-care visits.
- Clinical notes and other documentation due within 48 hours of visit.
- Maintain timely communication with supervisor, intake, and office.
- Be generally responsive during normal business hours.
- Territory includes Sacramento, CA, with potential expansion as needed.
- Schedule consists of 40 hours per week, Monday-Friday, with one weekend a month rotation.
- Training includes 3 full days, one in-house and two in the field.
- Documentation system used: Kinnser.
- Equipment issued includes an iPad and Verizon phone.
- Mileage reimbursement available for travel from first to last patient.
- Option for a company car is available with conditions.
- Productivity requirement of 30 points per week.
- Caseloads and visit mixes per day average 8 hours per day and 40 hours per week.
