Job Description
Job Title: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)
Job Specialty: Home Health
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: No
Experience: Minimum 1 year of Adult Home Health or Hospice experience
License: Must have license in hand
Certifications: CPR, BLS
Must-Have: Proficient in HomeCare HomeBase EMR, and completed a retraining course within the last 6 months if not used recently
Job Description:
- Provides, coordinates, and directs home nursing care according to physician orders based on agency policies and procedures.
- Responsible for supervising and evaluating Home Health Aides and Licensed Practical/Vocational Nurses.
- Responsible for home visits and periodic office meetings. Must have reliable transportation, a current driver's license, and required liability insurance.
- The average number of patients per day ranges from 5 to 8, with productivity goals of 30 points per week.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- No first-time candidates; must have recent specialty experience.
- Weekend requirements vary per location and should be discussed during the interview.
- On-call duties vary per location and profession.
- Must be willing to travel up to 75 miles from the home clinic, with mileage reimbursed at the IRS standard rate for travel between patient homes.
- Candidates will be visiting both patients in care facilities and their homes.
- Possibility of hiring the candidate after the contract ends, pending performance.
- Proficient use of EMR systems such as HomeCare HomeBase, Outlook, Teams, and Forcura.
- Documentation must be completed within 24 hours of the appointment with the patient.
- Required to accept every patient on the schedule, regardless of their location relative to the previous appointment.
- Candidates may need to float to other nearby clinics if needed to meet their financial needs.
- Regular internal meetings every Wednesday in the office at 0830, approximately 1 hour long.
