Job Description
Job Title: Floor Nurse
Job Specialty: Specialty Care
Job Duration: 13 months
Shift: Monday to Friday
Guaranteed Hours: 40 hours per week
Experience:
- At least 1 year of general nursing experience with at least 6 months as a clinical nurse in an outpatient clinic OR at least 2 years of general nursing experience with a minimum of 1 year in an ambulatory setting or 1 year of case management experience.
License: Current full, unrestricted Registered Nursing license in Alaska required.
Certifications:
- Basic Life Support certification preferred.
- National Case Management certification preferred.
Must-Have: Proof of flu vaccine required.
Job Description:
- Responsible for the active management of patients.
- Collaborate with medical providers and interdisciplinary team members to establish individualized medical goals that maximize patient healthcare outcomes.
- Coordinate multi-disciplinary care conferences for high-risk or complex patients referred to the case management process.
- Manage acute and chronic care needs, as well as health maintenance, by reviewing EHR reminders, and providing outreach and education to patients.
- Participate in case management reviews with a multi-disciplinary team of providers for high-need patients.
- Advocate for patient/family decision-making, independence, self-care, growth, and development.
- Address patient needs in a prompt and efficient manner.
- Facilitate the coordination of patient referrals within the organization and to outside facilities.
- Track the progress of referrals and outcomes of care, and provide periodic status reports to providers.
- Perform staff nursing duties including assessment, documentation, education, and administering medications/therapies.
- Maintain annual competencies, including safety and infection control requirements.
- Build bridges and create rapport with team members across the organization.
- Participate in department and clinic team improvement efforts.
- Act as a leader among nursing staff and unit coordinators.
- Participate in the continued development of the case management role in primary care.
- Participate in triage roles and respond to emergency situations.
- Facilitate the coordination of specialty clinic referrals and manage referrals and waitlists.
- Act as the primary nurse in specialty clinics, providing clinical support and preparing clinical spaces.
- Assist medical staff with educational in-services provided by specialty clinic providers.
- Manage OB case management, including education, outreach, and tracking processes for OB patients.
- Coordinate video teleconferencing visits with specialty providers and manage Exercise Treadmill Tests referrals and appointments.
- Perform other duties as assigned, including special projects necessary for providing comprehensive health services.
