License: A valid, current, full, and unrestricted RN license in the State of Alaska
Certifications: Basic Life Support certification
Must-Have: Active Alaska RN License, Flu Vaccination

Job Description:

- Responsible for the active management of patients, collaborating with medical providers and all interdisciplinary team members to establish individualized medical goals that maximize patient healthcare outcomes.
- Utilize problem-solving, critical thinking, and anticipatory skills to meet the needs of the patient and provider in a prompt, efficient manner.
- Work closely with providers and other clinical staff to facilitate and coordinate patient referrals to Surgery/Specialty Clinic.
- Review referral records to ensure all appropriate information is available for provider review and track the progress of referrals and outcomes of care, providing periodic status reports to providers on outstanding referrals.
- Serve as the primary clinical contact regarding patient care and care coordination, proactively identifying delays and obstacles to care and working to remedy individual cases and improve the process.
- Perform staff nursing duties in the various nursing roles in the clinic, including assessment, documentation, education, and administering appropriate medications and/or therapies.
- Maintain annual competencies including safety and infection control requirements, performing duties in a responsible, respectful, and caring manner to promote effective nursing staff relationships.
- Ensure a clean and safe working environment, including adequate supplies and upkeep of the unit and equipment, reporting any defective or missing equipment and safety hazards. Adhere to infection control policies and procedures.
- Work collaboratively by building bridges and creating rapport with team members within departments and across the organization.
- Participate in department and clinic team improvement efforts.
- Perform other duties as assigned, including special projects deemed necessary to provide comprehensive health services.
