Job Responsibilities

Serves on the Assertive Community Treatment (ACT) team and the Long -Acting injection (LAI) team providing evidence-based services in intensive non-residential treatment and rehabilitative services in accordance with the fidelity model, and in the clinic as assigned. This is a single point of responsibility for treatment, rehabilitation, and support needs for patients with Serious Mental Illness (SMI) who require higher levels of community care and have been well supported in the lower level of care options. Serves as the RN case manager for patients in Medication Management Only (MMO) status.
  • Provides direct services to, but not limited to, ACT patients, LAI patients, assessing the health, social, psychiatric and medical needs of Center patients.
  • Completes clinical documentation using collaborative documentation and ensures services are documented in accordance with SCDMH/QA requirements.
  • Engages in health promotion, prevention, and education activities.
  • Ensures patients are seen in accordance with their need (medical/psychiatric appointments.

Minimum and Additional Requirements

Graduation from an accredited school of nursing. A bachelor's degree in nursing and 2 years' experience; or a three-year diploma and 2 years of experience; or an associate degree in nursing and 2 years' experience as a registered nurse, of which 1 must have been in a community mental health program
