### Job Title:

Day Shift, Monday to Friday, 5x8-Hour (08:00 - 17:00)

### Experience:
Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience

### License:
Must have a current valid license

### Certifications:
CPR, BLS

### Must-Have:
- Proficiency with HomeCare HomeBase EMR, used within the last 6 months or completed a retraining course
- Reliable transportation, a valid driver's license, and liability insurance
- Willingness to travel up to 75 miles from the assigned home clinic
- Understanding that all documentation must be completed within 24 hours of the patient appointment

### Job Description:
- Coordinate and direct the provision of home nursing care according to physicians' orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses.
- Conduct home visits with an average of 5-8 patients per day.
- Handle various types of cases, including Start of Care, Wound Care, and Follow-up Visits.
- Participate in periodic office meetings.
- Attend to territory coverage in specific areas.
- No mandatory weekends, but voluntary work is possible.
- On-call requirements vary by location and profession.
- Complete weekly productivity expectations with 35 units of productivity.
- Local candidates accepted with no pay rate alteration.
